1. Summary

  • The Project had a representation at the National level in the Dissemination meeting of the Common Review Mission as well as in the Consultation on JSY.

  • The Project was actively involved in the District Health Action Planning workshop for Sagar and Rewa divisions on request of the State.

  • Orientation of the new NRHM reporting formats has been initiated in Chhatarpur district.

  • Core competency skills training has been initiated in Panna district.



2. Development in the field / Project area


A. Dissemination of Common Review Mission

  •  

  • The dissemination meeting of the Common Review Mission was held at Vigyan Bhavan, New Delhi, on 22nd December 2009.
  •  Dr. Yoichi Yamagata, Project Manager; and Dr. Aboli Gore, HRM Consultant, who were members of West Bengal and Haryana teams, participated in the meeting.
  •  State wise reports were presented and the Mission Directors of NRHM shared their responses towards the CRM findings.



B. Consultation on Janani Suraksha Yojana (JSY)

  • A consultation was held on 23rd December 2009 at New Delhi, to review the problems faced in implementation of the JSY scheme and to gather suggestions for strengthening the scheme.
  • The meeting was chaired by Ms. K. Sujatha Rao, Secretary, Health & Family Welfare.
  • The participants included Government officials, officials from NHSRC, NGOs and academic institutions. Dr. Aboli Gore from JICA Project attended the consultation.
  • After the consultation, the Secretary suggested that a Task Force should be formulated to work towards not only strengthening the JSY scheme, but to also address in an integrated way, other issues of care in pregnancy and related areas of child health.
  • Dr. Aboli Gore is a member of the National Task Force.



C. Orientation/ Planning workshop on District Health Action Plans (DHAP) (Sagar and Rewa)

  • On request of the DoHFW, GoMP, JICA team comprising of Dr. Yoichi Yamagata, Project Manager; Mr. S. Fareed Uddin, TQM Consultant and Project Operations Manager; Dr. Aboli Gore, HRM Consultant; and Ms. Richa Som, HMIS Consultant, facilitated the two days workshop for DHAP planning for:
  • Sagar -17th – 18th December 2009 for all the 5 districts of Sagar division
  • Rewa - 19th – 20th December 2009 for 3 districts of Rewa division.


D. Orientation on NRHM Reporting Formats in Chhatarpur

District level

  • The district level orientation on the new NRHM reporting formats was organized by Chhatarpur district on 2nd December 2009. JICA team comprising of
    Ms. Rohini and Ms. Dimple Save (Capacity Development Coordinators), facilitated the orientation.
  • The participants included BEE, DEO and BPM of all the 8 blocks. The DPM,
    Mr. Rajendra Khare and District Data Assistants Mr. Ritujit Singh and Mr. Neeraj Khare, were also present during the orientation.


Block level

  • JICA team facilitated orientation on the new NRHM reporting formats in Rajnagar block of Chhatarpur district on 3rd December 2009.
  •  The participants included ANMs, MPWm, LHVs and MPS from the field and BEE, DEO, BPM and other block staff.

E. Core Competency Skills training in Panna

  • A six days Core Competency training was designed by the project and tried in Damoh and Tikamgarh in 2006-2008.
  • However, due to logistics and other constraints, it was difficult to scale it up throughout the state. Hence, it was decided to unpack the 6 days training and make the slot of core competency skills for 2 days.
  • On 9th and 10th December 2009, the first batch of 2 days Core Competency Skills training was facilitated by the Project team in Amanganj block.
  • The participants included ANMs, MPWm, LHVs and MPS of the entire block.

3. Meetings / Seminars / Workshops

  • Dr. Yoichi Yamagata, Project Manager; and Mr. S. Fareed Uddin, TQM Consultant and Project Operations Manager, attended the meeting on planning process in NRHM office on 30th December 2009. Ms. Gitanjali Agarwal, Representative from Government of India, was present during the meeting. The meeting was attended by other Development Partners apart from the State officials.

  • The State IEC core group meeting was held at the NRHM office on 30th December 2009 to review the progress made in IEC for NRHM and to decide on new interventions in the current plan and the new IEC plan of 2010. The meeting was chaired by Dr. S.K.Shrivastava, Joint Director, NRHM/RCH. The participants included staff of state IEC bureau and development partners – UNICEF, UNFPA, JICA and NIPI.

  • Dr. Yoichi Yamagata, Project Manager; and Mr. S.Fareed Uddin, TQM Consultant and Project Operations Manager, met the Health Secretary to share the Project initiatives and progress.

  • Dr. Yoichi Yamagata, Project Manager attended the MEDP Closing workshop on Midwifery skills, organized by IIM Ahmedabad, from 15th – 16th December 2009 at New Delhi.

  • Dr. Yoichi Yamagata, Project Manager, visited NIHFW, New Delhi, and observed the ongoing GIS training. He met Dr. Deokinandan, Director, NIHFW, and discussed about GIS mapping.

  • Dr.Yoichi Yamagata, Project Manager, attended the Development Partners’ meeting held in UNFPA office, New Delhi, on 9th December 2009.

  • The National team from MoHFW organized the state and district officials on District Health Plans on 7th December 2009. This was followed by a meeting with the Health Secretary to update him about the planning process. JICA Consultants

  • (Dr. Aboli Gore, HRM Consultant; and Ms. Richa Som, HMIS Consultant participated in the workshop as well as the meeting.

  • Dr. Yoichi Yamagata, Project Manager, participated as observer in the meeting organized by the Government of India in New Delhi on 5th December 2009, on request of the Health Commissioner. The Mission Directors, NRHM, of all the states were called to discuss the HMIS for NRHM/RCH II.

  • Dr. Yoichi Yamagata, Project Manager; and Ms. Richa Som, HMIS Consultant, attended the meeting called by the Health Commissioner on 2nd December 2009. The Commissioner requested JICA Project to facilitate GIS mapping of 31 difficult districts identified by the Government of India.

Monthly Progress Report November 2009



1. Summary

  • The second Joint Coordination Committee was held on 11th November 2009 to review the progress of the Project interventions.

  • The Project was an active member of the third Common Review Mission under NRHM held from 3rd to 10th November 2009.

  • Maternal Health Cards have been introduced in Panna and Chhatarpur districts. Block wise orientation was held in both the districts.

  • Orientation on the new NRHM reporting formats have been initiated in Tikamgarh and Sagar districts.

  • Block level orientation on Bio Medical Waste management was initiated in Panna and Chhatarpur districts.

  • The Experts from NHSRC visited the Project area to observe the Project interventions.



2. Development in the field / Project area


A. Joint Coordination Committee

  • The second meeting of Joint Coordination Committee for the Reproductive Health Project in the State of Madhya Pradesh (Phase 2) was held on 11th November 2009 to review the achievement since the last JCC meeting, and to share the future action plan.

  • The DoHFW officials present for the JCC included Mr. S.R.Mohanty, Principal Secretary; Dr. Manohar Agnani, Health Commissioner; Dr. S.K.Shrivastava, Joint Director, RCH; Deputy Directors - Dr. Archana Mishra and Dr. Jayshree Chandra; and State Consultants for NRHM. The district team comprised of CMHO, and DPM from 5 districts of Sagar division.

  • Comments of the Health Commissioner:
    “JICA team is the best and has a lot of solidarity and coordination amongst them, due to the strong leadership of Dr. Yamagata”. He showed confidence in the Project interventions and suggested the following interventions should be scaled up in the entire State on priority basis with the Project support:

    1. Village Health & Nutrition Day (VHND)

    2. Bio Medical Waste (BMW) Management

    3. Health Management Information (HMIS)

    4. SBA training

    5. Maternity Wing organisation

B. Common Review Mission

  • The third Common Review Mission under NRHM was held from 3rd – 10th November 2009 for 17 States of India. The Government of India teams visited each state during the given period.

  • Dr. Yoichi Yamagata, Project Manager, was a member of the CRM team visiting West Bengal, while Dr. Aboli Gore, HRM Consultant, joined the Haryana team.

C. MH Card Orientation in Chhatarpur and Panna districts

  • In Chhatarpur, as in Sagar, the ANMs have not been trained on the Core Competency skills. Whereas in Panna, only 15 ANMs have been trained on Core Competency in August 2009. Hence, the methodology adopted for the orientation was the same as that in Sagar.
  • A total of 406 health workers of Chhatarpur and 214 of Panna including ANMs, MPWm, LHVs and MPS were oriented on the use of the MH card (see table below).
  • It was decided that the health workers would start filling MH cards for the new ANC registrations and for the SHC village to start with.

D. Block level Orientation on NRHM Reporting Formats in Sagar

  • JICA team facilitated orientation on the new NRHM reporting formats in Rahatgarh block of Sagar district on 19th November 2009.
  • The participants included ANMs, MPWm, LHVs and MPS from the field and BEE, DEO, BPM and other block staff.
  • The BMO took lot of interest and was present throughout the orientation.

E. District level Orientation on NRHM Reporting Formats in Tikamgarh

  • The district level orientation on the new NRHM reporting formats was organized by Tikamgarh district on 20th November 2009. JICA team comprising of Ms. Richa Som, HMIS Consusltant and Mr. Kamlesh Mishra, Capacity Development Coordinator, facilitated the orientation.
  • The participants included BEE, DEO and BPM of all the 6 blocks. The Civil Surgeon, Dr. Sunit Jain; and I/C DPM, Mr. Sunil Verma, were also present during the orientation.

E. Block level orientation on Bio Medical Waste management in Panna and Chhatarpur

  • An orientation program on Bio Medical Waste (BMW) management was organized in 2 blocks of Panna and Chhatarpur districts each.
  •  In Panna, the orientation was held in Amanganj block on 3rd November 2009 and in Gunnor block on 23rd November 2009. Whereas, in Chhatarpur, the orientation was held in Nowgaon block on 24th November 2009 and in Rajnagar block on 27th November 2009.
  • The participants included Staff Nurses, LHV, ANM, Ward boys, Ayahs and Sweepers.

3. Meetings / Seminars / Workshops

  • The IX ICM Asia – Pacific Midwives’ Conference on “Setting Standards for Safe Childbirth Empowering Women, Enabling Midwives” was held from 19-22 November 2009 at Hotel Marriott, Hyderabad.

  • JICA Project team comprising of Dr. Yoichi Yamagata, Project Manager;
    Ms. Mari Tsuda, Project Coordinator; Ms. Rohini, Training Coordinator; and
    Ms. Kana Shimoda, Japanese Volunteer attended the conference. The Project also sponsored 5 participants from the Project area (one from each district) with the objective to introduce them to the new developments in midwifery practice and provide an opportunity to share their experiences.

4. Visitors

NHSRC team visit to Project area

  • Dr P Padmanabhan, Advisor (Public Health Administration) and Mr. Prashantha K S Consultant (Public Health administration) from NHSRC, New Delhi, visited the Project area from 25th to 26th November 2009 to observe the Project interventions.

  • Dr Yoichi Yamagata, Project Manager; Ms Richa Som, Consultant HMIS and Dr Aboli Gore, Consultant HRM, accompanied the team.

Monthly Progress Report October 2009



1. Summary

  • The Project team was invited for the “Bio Medical Waste Management” (BMW) workshop organized by RO, MPPCB, Gwalior.

  • Block level BMW orientations have been initiated in Tikamgarh and Sagar districts.

  • Block Medical Officers of Tikamgarh district have been oriented on “GIS as a tool for EBPM”.

  • Japanese Volunteers (3) visited Project site as part of their induction training.



2. Development in the field / Project area


A. Supervisors’ Orientation in Tikamgarh

An orientation program for field Supervisors of Tikamgarh district was held on
5th October 2009 to stress upon the importance of their roles and responsibilities in implementation and monitoring of field level activities.
The objective of the orientation was to motivate and revitalize the Supervisory cadre. This was done after the encouraging results of a similar program in Damoh district in July 2009.

B. Bio Medical Waste Management Orientation at Block level in Tikamgarh

  • An orientation program on Bio Medical Waste (BMW) management was organized at CHC Palera on 6th October 2009. Dr. Arya, Block Medical Officer, took the lead and JICA Consultants facilitated the orientation.
  • This orientation was a sequel to the “Awareness & Planning Workshop on Bio Medical Waste Management (BMW)” held in Sagar on 11th September 2009.
  • The participants (20) included Staff Nurses, LHV, ANM, Ward boys, Ayahs and Sweepers.

C. Bio Medical Waste Management workshop in Gwalior division

  • The Regional Office, MP Pollution Control Board, Gwalior organized a workshop on
    “Bio Medical Waste Management” on 9th October 2009, wherein Government Medical Officers, Staff Nurses, other para-medical staff, as well as Doctors from Private Nursing Homes & Hospitals of Gwalior division were invited.
  • JICA/MP-RH Project team was specially invited for the workshop. Dr. Yoichi Yamagata, Project Manager, was the Guest of Honour, while Mr. S. Fareed Uddin, Consultant – TQM was invited as a Guest Speaker.

A. Development Partners’ meeting

  • Dr. Yoichi Yamagata, Project Manager, attended the Development Partners’ meeting in New Delhi on 14th October 2009.

  • He showed the video on Village Health & Nutrition Day (VHND) developed by the Project to the group.


B. Technical Review of IPC Trainings in Madhya Pradesh

  • A State level consultation was organized by UNICEF Madhya Pradesh from
    22nd to 23rd October 2009, to review the IPC/Communication materials available in the State and to develop a time bound action plan for strengthening the training network in MP.

  • Dr. Yoichi Yamagata, Project Manager, was invited as the Guest of Honour.


C. Inauguration of CPHE

  • Dr. Yoichi Yamagata, Project Manager, and Mr. S. Fareed Uddin, Project Operations Manager and TQM Consultant, attended the inaugural function of the Centre for Public Health and Equity on 29th October 2009.

  • Dr. Thelma Narayan is heading SOCHARA, Bangalore, which is starting a Fellowship Program on Public Health in MP. They are very impressed by the Project interventions and would like to place 3 Fellows in one district each of Sagar division.

A. Sight Savers’ International team

  • The State team of Sight Savers International comprising of Ms. Archana Bhambhal, State Representative and Ms. Jayshree Kumar visited the Project office on
    7th October 2009.



B. Sight Savers’ International team


  • The 3 Japanese Volunteers namely Ms. Kana Shimoda (Midwife), Mr. Yuta Kono (Reproductive Health Extension Educator) and Mr. Yuta Yamauchi (Reproductive Health Extension Educator) visited the Project site – Bhopal and Damoh from 19th to 21st October 2009 for induction to their new posting.



C. JICA India Officials


  • Mr. M.P.Singh, Senior Development Specialist, JICA India Office, visited the Project office on 30th October 2009.

  • He met the Principal Secretary Health to finalise the date for Joint Coordination Committee meeting and to request him to follow up for Project Phase 3 application at GoI level.

1. Summary

  •  The Project Extension Application was dispatched to the Ministry of Health & Family Welfare, New Delhi.

  • Maternal Health card orientation has been completed in Sagar district.

  • JICA Project team facilitated the workshop on Bio Medical Waste Management organized by MPPCB, Sagar, on 11th September 2009.

  • Dr. Yoichi Yamagata, Project Manager, participated in the workshop on “Capacity Building of CMHOs” organized by the Department of Health & Family Welfare on 18th and 22nd September 2009.

A. Application for Project Extension

  • The Project Extension Application for Phase 3 was signed by the Health Commissioner, Department of Health & Family Welfare, GoMP, on 11th September 2009 and dispatched to the Ministry of Health & Family Welfare, New Delhi.



B. Maternal Health (MH) Card orientation in Sagar

  • The MH card orientation in Sagar district for the health workers was completed in this month as shown in the table below:

C. Bio Medical Waste Management Workshop

  • MP Pollution Control Board, Regional Office, Sagar organised an “Awareness & Planning Workshop on Bio Medical Waste Management (BMW)” on 11th September 2009 in Sagar for the District Chief Medical & Health Officers (CMHOs), Medical Officers of the Block and Sector hospitals, Staff Nurses, Ward Boys and Sweepers of from the Government sector, public sector as well as private hospitals of Sagar division. Municipal Officers were also invited.

  • Role of JICA/MP Reproductive Health Project:
    RJD Office, Sagar – to liaison with the RJD office to ensure participation from all
    the 5 districts
    MPPCB, Sagar – to provide technical support for designing the workshop agenda
    and providing reading material

  • Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP, through his presentation, stressed on the importance of Quality and Quantity taking a Simile of the film Sholay.

  • Mr. S. Fareed Uddin, Consultant TQM and Project Operations Manager, JICA/MP-RHP presented the implementation aspects of BMW Management in Health facilities.

D. Divisional Meeting at Sagar

  • JICA Consultants attended the Divisional Meeting called by the Regional Joint Director, Health, Sagar division, on 2nd September 2009.

  • All the 5 CMHOs and DPMs were present for the meeting.

  • JICA Consultants discussed the following issues:

    • Printing of the new NRHM Reporting Formats at district level

    • Orientation of the new NRHM Reporting Formats at block level

    • MH card orientation at block level, specially in Chhatarpur and Panna districts

  • The CMHOs and DPMs confirmed that they would finalise the dates for orientation of the MH card as well as for the reporting formats at the earliest.



E. Capacity Building of CMHOs

  • The State Health Minister chaired the workshop organized by the Department of Health & Family Welfare, in 2 batches, one on 18th September 2009 in Gwalior (Gwalior, Jabalpur and Rewa divisions) and the second on 22nd September 2009 in Sehore (Bhopal, Indore, Ujjain and Sagar divisions).

  • Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP participated in both the workshops.

A. BCC Core Group Meeting

  • The Health Commissioner, DoHFW, had called for a meeting on 1st September 2009 to finalise the BCC strategic actions for NRHM and streamlining the terms of reference of the BCC Working Group.

  • The participants included Dr. S.K.Shrivastava – JD RCH/NRHM, DoHFW;
    Dr. Dhanila Narayan – I/C IEC Bureau; Mr. Ashish Choubey – State IEC Consultant, NRHM and Mr. Manoj Choudhary – Consultant, IEC Bureau and Development Partners of the State. JICA Project is a part of the Core Group.

  • Mr. Mitsunori Saito, Representative, JICA India Office, visited Madhya Pradesh on 8th September 2009. He also met the Principal Secretary, DoHFW, to discuss on the plan for video shooting in the Project area for multi media teaching materials.

Monthly Progress Report August 2009

1. Summary

  • The JICA/MP-RH Project Extension file for Phase 3 was approved in the State Health Society meeting chaired by the Chief Secretary on 4th August 2009.

  • Core Competency Training of ANMs was initiated in Panna from 31st August 2009.

  • Maternal Health card orientation was initiated in Sagar district from 31st August 2009.

  • JICA Consultant participated in the National Consultation on GIS Mapping organized by the Ministry of Health & Family Welfare in collaboration with WHO and IIHMR, New Delhi, on 31st Aug & 1st September 2009.

2. Development in the field / Project area

A. Core Competency Training in Panna

  • The first batch of the 6 days Core Competency Training of ANMs began on 31st August 2009. There were a total of 15 ANMs from Amanganj, Devendranagar and Pawai blocks.

  • JICA team (Dr. Aboli Gore and Mr. Kamlesh Mishra) were present throughout the training to 1) establish the training methodology, 2) monitor the quality of the training and 3) coordinate for the smooth running of the training.

  • The training was conducted by Dr. D.K.Gupta, Ms. Kusum Gupta, Tutor, and Ms. Poornima Verma, Staff Nurse, District Hospital. The specific technical sessions were taken by Experts from the district, which included Dr. Maya Pandey, Gynecologist; Dr. Meena Namdeo, Gynecologist; and Dr. Vinchurkar, Civil Surgeon and Dr. L.K.Tiwari, Paediatrician.

  •  Dr. D.K.Gupta, District Training Officer, coordinated the entire training and also took some sessions during the training.

  • The ANC modules developed by Government of Madhya Pradesh were used for the training.

  • The Project provided kit bag, BP instrument, stethoscope and ANC skill checklist to all the participants.


B. Maternal Health (MH) Card orientation in Sagar

  • The Directives for the use of MH card was issued by the Joint Director, RCH/ NRHM, Department of Health & Family Welfare, GoMP, vide letter nos. RCH/NRHM/MH/R009/180, dated 22nd July 2009. In response to this, the CMHO of Sagar district issued a letter to the BMOs of all 11 blocks of Sagar to organize orientation of all the block staff on MH card.

  • The CMHO invited JICA team to conduct the orientation.

  • The one day MH card orientation in Sagar district started with Agasod block. The orientation was held at Bina Civil Hospital. A total of 31 ANMs, MPWm, LHV and MPS attended the orientation.

  • In Sagar, the ANMs have not been trained on the Core Competency skills. Hence, a different methodology was adopted by the Project for the orientation.

  • Emphasis was laid on the “Mock session”, wherein the participants were asked to fill up the MH card in a field like situation. This was done to ensure proper understanding and involvement of the participants.

  • The remaining 10 blocks of the district would be covered during the month of September 2009.

C. Orientation on new NRHM Reporting formats

  • JICA Project has been providing Technical Assistance to the Department of Health & Family Welfare, GoMP, for implementing the new NRHM Reporting Formats across the State (For details, see JICA MP RHP TA for NRHM Reporting Formats).

  • As a follow up of the State level Orientation on NRHM Reporting Formats organized by the Department of Health & Family Welfare in March 2009, a letter was issued to all the CMHOs by the Joint Director RCH/NRHM, DoHFW, to get the formats printed for the first quarter and start using them.

  • Mr. Jaimon Thomas, DPM, Damoh, took an active role in consultation with JICA Project, for rolling out the reporting formats in the district.

  • JICA Consultants facilitated the first orientation in Jabera block on 10th August 2009 to set the methodology.

  • The participants included ANMs (26), MPWs and MPS (10) and LHVs (2). The BMO, DEO and BEE were also present during the orientation.


D. Planning for Bio Medical Waste Management Workshop

  • In response to the letter from MP Pollution Control Board, Regional Office, Sagar, 1175/RO/PCB/BIO/09 dated 29th July 2009, JICA Project gave technical assistance for the organization of the “Awareness & Planning Workshop on Bio Medical Waste Management (BMW)” to be held on 11th September 2009 in Sagar.

  • JICA consultant visited the MPPCB office in Sagar on 24th August 2009 to facilitate the planning and designing of the workshop.

  • Role of JICA/MP Reproductive Health Project:
    RJD Office, Sagar – to liaison with the RJD office to ensure participation from all
    the 5 districts
    MPPCB, Sagar – to provide technical support for designing the workshop agenda
    and providing reading material


E. SBA training

  • The tutors of Damoh and Tikamgarh districts are conducting the SBA training at district level as a part of the district PIP.

  • JJICA Coordinator visited both the districts during the month to observe the quality of ongoing SBA training.

  • It was observed in both the places that the training was conducted as per the sessions planned; the tutors maintained the time for clinical as well as classroom sessions; and tools developed by the Project were being used during the classroom session, as per the TOT organized by JICA Project on the training methodology for improving the quality of SBA training from 22nd – 24th July 2008 in Bhopal.

  • In ikamgarh, the tutors are also maintaining daily record of the training separately.
    Issue of concern

  • The vial of Magsulf injection should be of 10 ml (the woman has to be given 20 ml). However, currently each vial contains only 2 ml.


3. Meetings / Seminars / Workshops


A. National Consultation on GIS Mapping

  • A National Consultation on GIS Mapping of Health facilities and services was organized by the Ministry of Health & Family Welfare in collaboration with WHO and IIHMR, New Delhi, on 31st Aug & 1st September 2009.

  • Mr. S. Fareed Uddin, Consultant Total Quality Management (TQM), JICA/MP-RHP; and Mr. Yoshioka Kota, Student from the Nagasaki University, on internship in the Project, were invited to participate in the Consultation.

BB. Meeting in JICA Headquarter


  • Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP is one of the authors of the book titled, “Senior Advisor of JICA: Facilitators of Technical Cooperation”. He gave a lecture in JICA IFIC, Ichigaya, Japan, on 24th August 2009, and shared his experiences with the group.



C. Workshop on Community Radio

  • JICA Coordinator attended an orientation workshop on understanding the potential of community radio in contributing to social and economic development was organized jointly by UNICEF and VANYA (Tribal Welfare Department) from 26-27 August 2009 at Bhopal.



D. Meeting on Promotion of Breastfeedingbr />

  •  JICA Coordinator participated in the meeting organized by UNICEF to review the Communication Strategy and Media Plan on Promotion of Early Initiation of Breastfeeding, Colostrum feeding and Exclusive Breastfeeding in Madhya Pradesh on 25th August 2009. JICA Coordinator shared the Project experiences and gave the following suggestions:

    •  Orientation/Training of maternity wing staff to ensure early initiation of BF and colostrum feeding.

    •  Counselling of all pregnant women, and their family members on early initiation of BF and colostrum feeding during ANC check ups, Village Health & Nutrition Day (VHND) and Mangal Diwas.

    •  Counselling of all lactating women on exclusive BF during VHND and Mangal Diwas.


4. Visitors

  •  Mr. Fumitoshi Yano, JOCV Program Coordinator, JICA India Office, alongwith
    Ms. Keiko Haneoka, visited Madhya Pradesh from 10th – 13 August 2009. Ms. Mari Tsuda, Project Coordinator, JICA/MP-RHP accompanied them to Damoh and Tikamgarh districts to prepare for the working place and accommodation of the JOCVs (Japanese Volunteers). The other purpose of the visit was to brief
    Ms. Haneoka on the Project activities, since she would be taking charge as the new JOCV Program Coordinator.

Performance in Numbers

1. Summary

  • Dr. S.K.Shrivastava, Joint Director RCH, has issued directives to all the 5 CMHOs of Sagar division implement the use of Maternal Health Card developed by the State with technical assistance from JICA/MP-RHP.

  • The Project team facilitated the Supervisors’ orientation organized by the Department of Health & Family Welfare, Damoh, on 9th July 2009. The purpose of the orientation was to revitalize the supervisory cadre into the system. Supervisors have to play a vital role in implementation and monitoring of field level activities.

Tikamgarh

  • JICA Training Coordinator (Ms. Rohini Jinsiwale) visited 5 CHCs (Khargapur, Niwari, Jatara, Palera and Badagaon) and 4 PHCs (Mohangarh, Budera, Taricharkala, Digoda) to assess the quality of delivery services being practices and to provide on the job coaching on relevant areas.

  • JICA Documentation Coordinator (Ms. Dimple Save) visited SHCs of Niwari (4), Jatara (3) and Badagaon (2) blocks to assess the quality of services provided, with special focus on SHC organization, ANC clinic, proper and regular filling of Maternal Health card and orientation on Village Health & Nutrition Day (VHND). 

Damoh
Supervisors’ Orientation

  • Supervisors’ orientation was held on 9th July 2009, to revitalize the supervisory cadre into the system. Supervisors have to play a vital role in implementation and monitoring of field level activities.

  • A total of 43 participants (Male and Female Supervisors) attended the orientation program. The orientation was organized by the Department of Health & Family Welfare, Damoh, and was facilitated by JICA/MP-RHP team (Ms. Richa Som and Ms. Rohini Jinsiwale).

Panna

  • JICA team facilitated Maternity Wing organization at CHC Amanganj and oriented the Staff Nurses and the Gynecologist on the same.

  • Team Building exercise was conducted at CHC Amanganj.

  • JICA team initiated process for operationalization of NRHM reporting formats at all levels in the district.

  • The team observed 2 VHNDs in Amanganj block. It was observed that there was some conflict among the health and ICDS field workers. JICA team facilitated the conflict resolution amongst them. It was also observed that the information about organization of VHNDs had not reached upto the Anganwadi worker level.

  • The Block Medical Officer of CHC Amanganj, Dr. Choudhary, has started monitoring the VHNDs on every Tuesday.

Village Health & Nutrition Day (VHND)

  • During the month, the following number of VHNDs were held in Sagar division:
    Damoh – 304 (Tendukheda, Jabera, Hatta and Pathariya blocks)
    Tikamgarh – 37 (Prithvipur and Jatara blocks)
    Panna – 8 (Amanganj block)
    Chhatarpur – 12 (Rajnagar and Nowgaon blocks)

4. Meetings

  • Dr. Aboli Gore, HRM Consultant and Ms. Richa Som, HMIS Consultant, met
    Dr. Archana Mishra, Deputy Director, Maternal Health, on 24th July 2009, to discuss about further Project interventions.

  • A meeting was organized by Department of Women & Child Development and Department of Health & Family Welfare on 23rd July 2009, to discuss on Upgradation of AWCs to Village Health & Nutrition Centres. The participants included Dr. Jayshree Chandra, Deputy Director, Child Health, DoHFW;
    Dr. Archana Mishra, Deputy Director, Maternal Health, DoHFW; Mr. Praveen Gangrade, Deputy Director, DWCD; Development Partners – NIPI, MI, JICA/MP-RHP, MP TAST.

  • Dr. Yoichi Yamagata, Project Manager, attended the Development Partners’ meeting held on 15th July 2009 at the World Bank office, New Delhi.

  • Dr. Yoichi Yamagata, Project Manager, attended the Review Meeting of JRM 6 organised by the Secretary of Health, Government of India, on 7th July 2009, New Delhi.

  • JICA team (Dr. Yoichi Yamagata, Project Manager; Ms. S. Fareed Uddin, TQM Consultant; Ms. Richa Som HMIS Consultant; and Mr. Yoshioka Kota, Intern) had a meeting with the DoHFW officials on 3rd July 2009, to familiarize them on the utility of GIS (Geographic Information System). The participants included
    Dr. S.K. Srivastava, Joint Director, RCH; Dr. Jayshree Chandra, Deputy Director, Child Health, DoHFW; Dr. Archana Mishra, Deputy Director, Maternal Health, DoHFW; SDOs, DoHFW; Dr. Rajesh Singh and Dr. Reeta Mohan from MP TAST.

  • A meeting was organized by the NRHM office on 2nd July 2009, to 1) agree on the format and contents of the concept paper for introduction of fortified food products for prevention of micro nutrient deficiency in pregnant women and under-5 children, and 2) agree on the structure of presentation for the ministers of Health and WCD on implementation of new WHO standards of Growth monitoring. The participants included officials of DoHFW, Development Partners – MP TAST, UNICEF,
    JICA/MP-RHP and MI; and Dr. Amita Singh, Nutritionist.

5. Visitors

  • Dr. Sundararaman, Executive Director, NHSRC, New Delhi, visited the Project office on 16th July 2009 for finalization of the draft for Primary Health Registers.

  • Dr. Alka Chhadha from NHSRC, New Delhi, was on a visit to Madhya Pradesh in the first week of July 2009, to document about the Janani Express. She visited the Project office and met the team.

  • A Journalist from JICA’ s Monthly Journal titled “JICA’s World”, visited the Project office on 7th July 2009, to discuss about Dr. Yoichi Yamagata’s life history.

  • JICA Coordinator participated in the National Nutrition Consultation on 4th June 2009, organized by the Department of Women & Child Development, GoMP, to finalise the State Nutrition Policy.

Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP, participated in the International Task Force for Disease Eradication in Atlanta.

  • The Carter Centre, Atlanta, invited Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP, to participate in the International Task Force for Disease Eradication. The discussion revolved around exploring possibilities for eradication of Measles.

  • Dr. Yamagata also visited the Office of Population Research, Princeton University, and had discussions with Dr. Burton Singer on the usage of GIS (Geographical Information System) as a tool for scaling up of technical innovations.

JICA Consultants participate in the Review of Joint Review Mission (JRM)

  • Mr. S.Fareed Uddin, TQM Consultant and Project Operations Manager; and
    Dr. Aboli Gore, HRM Consultant, JICA/MP-RHP, participated in the briefing of the Joint Review Mission (JRM), Government of India, held at New Delhi from 15th – 18th June 2009.

National Consultation on Nutrition Policy for Madhya Pradesh

  • The Department of Women & Child Development, GoMP, with support from Madhya Pradesh Technical Assistance Support Team organized a one day National Consultation on Nutrition Policy for Madhya Pradesh, on 4th June 2009. The objectives of the consultation were:

    • To sensitize the policy makers of the concerned sectors towards the magnitude of the problem of malnutrition in Madhya Pradesh and the need for a State Nutrition Policy

    • To highlight the importance of strong governance and intersectoral coordination for malnutrition reduction

    • To identify priority strategies and actions for a State wide Nutrition policy and a State Plan of Action on Nutrition

  • The participants included Senior Officials from the key sectors of Government of Madhya Pradesh, namely, Women & Child Development, Health & Family Welfare, Education, Food & Civil Supplies, Rural Development and PRI, Tribal Welfare, Agriculture-Horticulture and Planning. Representatives of various Development Partners and NGOs were also invited.

  • Eminent Nutrition Experts invited for the consultation included –
    Dr. K. Vijayraghavan, NIN, Hyderabad; Dr. Mehtab Bamji; Dr. C.S. Pandav, AIIMS, New Delhi; Dr. G.N.V.Brahmam, NIN, Hyderabad; Dr. Purnima Menon; Dr. Ann Philpott, Health & Nutrition Advisor, DFID, India office; and Ms. Shashi Prabha Gupta, Nutrition Policy Advisor, MP TAST.

  • The Consultation was chaired by Ms. Tinoo Joshi, Principal Secretary, Department of Women & Child Development, GoMP. She welcomed all the Experts and participants and gave the opening remarks.

  • Ms. Kalpana Shrivastava, Commissioner, Department of Women & Child Development, GoMP shared the Special Initiatives taken by the Department in the recent past.

  • Dr. G.N.V.Brahmam, Senior Deputy Director and Head Community Studies, NIN, Hyderabad, presented an analysis on the Diet and Nutritional Status of the Rural Communities in the State of Madhya Pradesh.

  • This was followed by the presentation on “Strategies for Reduction in Severe and Acute Malnutrition in Madhya Pradesh” by Dr.S.K.Shrivastava, Joint Director, DoHFW.

  • The last presentation was made by Ms. Shashi Prabha Gupta, Nutrition Policy Advisor, M P TAST on “Towards the State Nutrition Policy for Madhya Pradesh”.

  • During the Open House, inputs were given by the Eminent Nutrition Experts on the draft Nutrition Policy; Implementation mechanism; Financial implications and Monitoring.

  • The Senior Officials from various departments also gave valuable suggestions as well as assured to provide support for the development of the State.

  • Ms. Dimple Save, JICA/MP-RHP, participated in the Consultation. She shared the Project experiences with the group and gave inputs during the Group Work.

4. Meetings

  • A meeting was called by the Micronutrient Initiative in collaboration with WCD and H&FW departments on 24th June 2009. The purpose of the meeting was to discuss on the role of micronutrients in Anaemia prevention, enriching complementary food & growth promotion .Feasibility to introduce fortified food items through the ICDS. The participants included officials from WCD, Health and Food & Civil Supplies department, team from MI India office, New Delhi; Development Partners including MP TAST and JICA.

The findings of Mid Term Review disseminated in Sagar.

  • A meeting of the officials of the Department of Health, Sagar division was held in Sagar on 13th May 2009, to share the Mid Term Report of JICA/MP-RH Project and the findings and the recommendations from the Review Mission, led by
    Dr. Tomohiko Sugishita.

  • The meeting was chaired by the Dr. K.K.Tamrakar, Regional Joint Director, DoHFW, Sagar. The participants included Divisional level Officials, CMHOs, DPMs and BMOs of all the 5 districts of Sagar division.

  • Dr. Yoichi Yamagata presented the Mid Term Report to Dr. K.K.Tamrakar, a copy of which was also distributed to the participants. The key points from the Report were then highlighted by Dr. Yamagata.

  • Dr. Yu Sasaki of JICA India office (member of the Mid Term Review Mission), presented the Findings and the Recommendations of the Review Mission.

  • Mr. Fareed Uddin introduced the Geographical Information System (GIS) to the participants and explained how it could be used as an effective monitoring tool. This was followed by the presentation by Dr. Yamagata based on his previous Project experience in Gautemala, Central America, for Control of Chagas disease.

  • Dr. Yamagata also presented the video films prepared by the Project on
    1) Village Health & Nutrition Day (VHND), 2) Maternity Wing organization, and
    3) ANC clinic, to the RJD. The films were exhibited towards the end of the meeting.

Orientation of Project staff on use of GIS

  • Mr. Uday Mahesh, Managing Director, Tectadum Info Services Pvt. Ltd., Hyderabad, visited Bhopal from 18th to 19th May 2009 to orient the Project Staff on the use of GIS and the GIS software.

4.Meetings

  • Dr. Yoichi Yamagata, Mr. S. Fareed Uddin and Dr. Aboli Gore met the Mr. Devraj Birdi, Principal Secretary, DoHFW and Dr. Manohar Agnani, Health Commissioner, DoHFW on 22nd May 2009,to finalise the initiatives taken by the DoHFW for providing services to the community, to be presented to the Chief Minister. This was a follow up of the meeting held on 4th May 2009.

  • Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP met Ms. Yumiko Asakuma, Senior Representative; Ms. Kazumi Homma, Project Formulation Advisor, and
    Dr. Yu Sasaki, Lead Development Specialist at JICA India office on 7th May 2009, to discuss about the Project management.

  • Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP attended the Development Partners’ meeting on 6th May 2009 at the World Bank office, New Delhi, for the preparation for JRM 6 to be held in the last week of May 2009.

  • A meeting was called by Mr. Devraj Birdi, Principal Secretary, DoHFW, on 4th May 2009, which was attended by State Program Officers, SPMU, Development Partners and MP TAST. The purpose of the meeting was to get innovative ideas from the floor to develop strategies for better service delivery in the Health sector. Dr. Yamagata made a presentation on the importance and usage of GIS (Geographical Information System). The Principal Secretary requested JICA to provide GIS training to all the DPMs of the State.

    4th to 22nd May 2009 for field testing of the Reporting and Recording formats, developed by NHSRC with technical support from JICA/MP-RHP. The field testing was done in Damoh and Tikamgarh districts.

JICA Consultants invited for the Second Expert Group Meeting

  • An Expert Group Meeting (EGM) was held in Manesar from 31st March to 2nd April 2009 to revise and finalise the SBA (Skilled Birth Attendant) Training Package.

  • The Ministry of Health & Family Welfare, Government of India invited JICA Project Consultants to share the Project experiences. Mr. S.Fareed Uddin, TQM Consultant and Dr. Aboli Gore, HRM Consultant attended the meeting. The other participants included technical experts from Medical Colleges and Nursing Colleges and various Development Partners like UNFPA, UNICEF and WHO.

  • Dr. Archana Mishra, Deputy Director Maternal Health, DoHFW, Madhya Pradesh, and Dr. Ajesh Desai, Director SIHFW, Gujarat, were the only participants invited from the State Health Department in the group.

  • Suggestions and inputs given by JICA Consultants during the first Expert Group Meeting held on 7th November 2008 have been incorporated in the latest version of the SBA Training Guidelines.

  • The participants were divided into 3 groups, which worked on: 1) SBA Training Guidelines, 2) Facilitators’ Guide for SBA training and 3) Handbook for SBA procedures. The Ministry of Health & Family Welfare is in the process of developing the SBA Teaching Aids Tool Kit. Dr. Himanshu Bhushan, Assistant Commissioner, Maternal Health, requested the Consultants to share a set of the Tool Kit developed by the JICA Project. He also informed that JICA Consultants would be actively involved in this process.

Intern from Nagasaki University

  • Mr. KOTA Yoshioka, student of Graduate School for International Health Development, Nagasaki University, Japan, joined the Project as a Long Term Intern on 20th April 2009. The duration of his internship would be 7 months. The major objectives of his internship include:

    • Administrative training

    • Research on Scaling up innovative Health interventions in the Project area

4.Meetings

  • Dr. Yoichi Yamagata, JICA/MP-RHP, along with Dr. Yu Sasaki and Ms. Homma from JICA India office participated in a meeting held between Mr. Ahmed Mohd. Prasad, the new Joint Secretary, RCH 2 and the Development Partners, on 28th April 2009 at India Habitat Centre, New Delhi.

  • Dr. Yoichi Yamagata attended the meeting on Recording and Reporting Formats held on 22nd April 2009 at NRHM building, Bhopal. The meeting was chaired by
    Dr. S.K.Shrivastava, Joint Director, RCH.

  • Dr. Yoichi Yamagata, alongwith Dr. S.K.Shrivastava, Joint Director, RCH, met
    Mr. Hidetoshi Irigaki, Representative JICA India office, and exchanged information about the recent developments on either side. Dr. Shrivastava specially mentioned about the establishment of the State Quality Cell, which will become a mechanism for the Quality Standard to other regions.

  • On 16th April 2009, Dr. Yoichi Yamagata met Mr. Birdi, Principal Secretary, DoHFW, and handed over the Mid Term Report.

  • Dr. Yamagata attended the Development Partners’ meeting, held on 8th April 2009 at World Bank Office, New Delhi. He shared JICA’s Zaiqa with the partners.

  • Dr. Yoichi Yamagata and Ms. Richa Som met Dr.S.K.Shrivastava, Joint Director, RCH, on 1st April 2009, to discuss about future actions, keeping the theme “Quality” for this fiscal year as the base, and to seek appointment for the video shoot.

5. Visitors

  • Dr. Ray Barman from NHSRC visited JICA Project office on 21st April 2009, to discuss about the establishment of SHSRC in the State.

  • Mr. Chris Hirabayushi, Deputy Director (Programs), UNICEF, New Delhi, who was in Madhya Pradesh to attend a meeting hosted by UNICEF Bhopal office, visited the JICA Project office on 21st April 2009 and met the staff. He shared his experience as JICA Expert in Indonesia with the Project Team.

Performance in Numbers

1. Summary

  • JICA Consultants participated in the second Expert Group Meeting on finalization of SBA Training Package, held in Manesar from 31st March to 2nd April 2009.

  • JICA Project was invited to orient the District level Data Managers of all the 50 districts on the new NRHM Reporting formats proposed by the Government of India (16th to 21st March 2009)

  • A two days State level IEC/BCC workshop was organized by the State IEC Bureau from 16th to 17th March 2009, where JICA Coordinator was one of the Facilitator.

  • An exchange visit of the Staff Nurses, LHVs, and ANMs from various health facilities of Damoh district to PHC Sarra (Block Tendukheda of Damoh district) was organized by Mr. Jaimon Thomas, District Program Manager, Damoh, on 14th March 2009. The purpose of the visit was to introduce the staff to the best practices of PHC Sarra and thereby strengthen the maternal health services in their respective health facilities.

  • Prof. Machiko Higuchi, Professor (Medical Anthropology), National College of Nursing, Tokyo, visited Madhya Pradesh from 23rd March to 1st April 2009. The purpose of her visit was to conduct Indepth Interviews and Focus Group Interview (FGI) with the ANMs of the JICA Project area to assess the motivation factors and their impact on the performance of the ANMs.

  • Ms. Akiko Hirano (Consultant), who was also a part of the Program Formulation Mission, visited Madhya Pradesh for the second time from 2nd to 8th March 2009 to assess the possible mechanism for upscaling

State level orientation workshop on NRHM Reporting Formats

  • A two days orientation workshop was organized by NRHM, GoMP and MP TAST from 16th to 21st March 2009. The purpose of the workshop was to orient the District level Data Managers (ASO, DPM, DDA) of all the 50 districts (in 3 batches) on the NRHM Reporting Formats newly proposed by the Government of India.

  • The State invited JICA Consultants to orient the district teams on the Reporting formats.

  • The State Data Officers (Mr. Neeraj Shrivastava and Mr. Anand Tripathi) coordinated the entire workshop.

  1. IEC/BCC

    State IEC/BCC workshop

  • The State IEC Bureau, DoHFW, organized a two workshop on IEC/BCC from 17th to 18th March 2009 at the NRHM Conference Hall, Bhopal. The purpose of the workshop was to develop District level IEC/BCC plans for the fiscal year 2009-2010. The District IEC Consultants and District Media Officers from all the 50 districts participated in the workshop.

  • The workshop was facilitated by Dr. Veena Sinha, In-charge, State IEC Bureau, DoHFW, GoMP and Mr. Ashish Choubey, State IEC Consultant, NRHM. The Experts included Dr.S.K.Shrivastava, Joint Director, RCH/NRHM; Dr. Archana Mishra, Deputy Director Maternal Health; and Dr. Jayshree Chandra, Deputy Director, RCH/NRHM. Among the Development Partners, UNICEF and JICA were invited to facilitate the workshop and provide technical inputs.

  • JICA team comprising of Dr. Yoichi Yamagata, Project Manager; Ms. Dimple Save, Documentation Coordinator; Ms. Mari Tsuda, Project Coordinator; and
    Mr. Arshad Khan, IEC Coordinator were present during the workshop.

  • During the workshop each division was given the responsibility for planning and presenting the IEC/BCC strategy. Ms. Dimple Save (Documentation Coordinator) JICA/MP-RHP facilitated the Group Work for Sagar division. She also shared the knowledge products developed by the Project with the participants.

  • Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP witnessed the presentation made by Sagar division. Taking this opportunity, Dr. Yamagata raised the following strategic issues:

    • Combination between contents and media

    • Message should be supported by the quality of services

  • Dr. Veena Sinha has asked JICA to provide the soft copy of all the IEC materials so that it can be replicated across the State.

Second Expert Group Meeting for SBA Guidelines

Exchange visit organised by Damoh district

  • An exchange visit of the Staff Nurses, LHVs, and ANMs from various health facilities of Damoh district to PHC Sarra (Block Tendukheda of Damoh district) was organized by Mr. Jaimon Thomas, District Program Manager, Damoh, on 14th March 2009. The purpose of the visit was to introduce the staff to the best practices of PHC Sarra and thereby strengthen the maternal health services in their respective health facilities. Sarra is a remote PHC of Tendukheda block in Damoh district. It is located at a distance of 80 km from the district headquarter and comprises of tribal population.

  • The team led by the DPM met Dr. Tulsa Thakur, Block Medical Officer (BMO), Tendukheda as well as both the ANMs (Ms. Geeta and Ms. Mamta) posted at PHC Sarra. The BMO explained the participants how the PHC was organized and made functional as per the IPHS norms. The participants then saw the well organized and functional maternity wing as well as the other areas of the PHC. The ANMs also explained the system of waste collection and disposal at the PHC to the participants.

  • The participants appreciated the efforts taken by their colleagues and have assured to make their health facilities equally functional.

  • Ms. Neha Gupta, District IEC Consultant, and JICA Block Coordinators
    (Mr. Imteyaz Ahmed and Mr. Shahwar Khan) were also present during the visit.

Meetings

  • Ms. Dimple Save (Documentation Coordinator) attended the meeting held at State IEC Bureau on 31st March 2009 to give inputs for the posters on PC PNDT Act as well as other health issues.

  • MP TAST members visited JICA office on 30th March 2009 to discuss and identify the areas for possible collaboration with JICA/MP-RHP, especially Maternal Health issues and VHND.

  • Dr. Yoichi Yamagata, Project Manager, attended the TV Conference held between JICA India office and JICA Tokyo office to discuss on the Mid Term Review and Program Formulation Missions.

  • Dr. Yoichi Yamagata, Project Manager, met Mr. Hidetoshi Irigaki, Resident Representative, JICA ID; Ms. Yumiko Asakuma, Deputy Resident Representative, JICA ID; and Dr. Yu Sasaki, Senior Program Officer, JICA ID to exchange views on Program Formulation post Mid Term Review.

  • Dr. Aboli Gore (HRM Consultant) attended the meeting called by the Mission Director NRHM on 19th March 2009 at New Delhi, to discuss and finalise the Primary Health Registers. The meeting was facilitated by NHSRC.

  • Dr. Manohar Agnani, Health Commissioner, called a meeting of the Development Partners, where a presentation of software called ChRIS developed by the former Commissioner of Jabalpur division was made. Dr. Yoichi Yamagata, Project Manager, attended the meeting.

  • Dr. Aboli Gore (HRM Consultant) attended the second meeting of the State Nutrition Core Group held on 3rd March 2009 at the NRHM office.

  • Ms. Bindu Verghese from NORAD, New Delhi visited JICA Project office and had a discussion with Dr. Yoichi Yamagata, Project Manager, about the Project interventions.

Visitors

    Professor Higuchi, accompanied by Ms. Mari Tsuda, Project Coordinator and
    Ms. Dimple Save, Documentation Coordinator, visited Damoh and Tikamgarh districts between 23rd March to 28th March 2009. In all, 4 In-depth Interviews were conducted in Damoh, while 6 In-depth Interviews were conducted in Tikamgarh. One Focus Group Interview was conducted in each district. Ms. Dimple Save was the Moderator for both, the Interviews, as well as, the FGIs, while JICA Project Block Coordinators, Mr. Shahwar Khan and Mr. Priyank Dubey were the Note-takers.
    According to the analysis done by Professor Higuchi, JICA training increased the confidence level of the ANMs, due to which there has been an improvement in the services provided for Safe Motherhood. The analysis also revealed that a strong need was felt by the ANMs for continuous education/training on issues like Nutrition, Family Planning, Counselling skills, Post Natal Care etc.

  2. Ms. Akiko Hirano (Consultant), who was also a part of the Mid Term Review Mission, visited Madhya Pradesh for the second time from 2nd to 8th March 2009 to assess the possible mechanism for upscaling.
    Ms. Hirano, accompanied by Ms. Rohini Jinsiwale, Training Coordinator, visited the State Nursing Council in Bhopal and discussed with the Registrar (Ms. Jayanti Chourasia) about the functioning of the Council. She also met Ms. Anju Bhadoria, State Nursing Officer, DoHFW as well as Dr. A.K.Chowdhary, Civil Surgeon, Bhopal to discuss about the status of pre-service and in-service trainings of ANMs.
    Ms. Hirano, along with Dr. Yoichi Yamagata, Project Manager; and Ms. Dimple Save, Documentation Coordinator, visited Sehore to assess the training system for ANMs in non-JICA intervened area.

1. Summary

  • The Project Mid Term Review was held from 16th to 25th February 2009. The Review Mission and the Program Formulation Team was led by Dr. Tomohiko Sugishita, Senior Advisor, JICA Tokyo.

  • Dr. Manohar Agnani, Health Commissioner, released the Project Mid Term Report during the review meeting on 23rd February 2009.

  •  The Project Review Mission and the Program Formulation Team visited Damoh (Project intervention area) from 18th to 20th February 2009, to observe the Project activities in the field.

  • JICA is an active member of the State Core Groups (Nutrition Core Group and Equity, Access and Community Mobilisation Group) formulated by Dr. Manohar Agnani, Health Commissioner, DoHFW, Madhya Pradesh.

Data collection for Mid Term Review

Ms. Mariko Homma, Consultant, (part of the Review Mission) visited Madhya Pradesh from 9th to 13th February 2009. The purpose of her visit was to visit the Project site and collect data for the Mid Term Review.
Ms. Dimple Save (Documentation Coordinator), accompanied Ms. Homma to Damoh for data collection. Ms. Homma interviewed 15 officials of various cadres (CMHO, DPM, 2 BMOs, 5 ANMs, 1 LHV, 1 MPS, 1 MPWm, 1 ASHA and 1 AWW) during her visit. She also observed a Village Health and Nutrition Day session at village Hinota.

Mid Term Review of the Project

The Mid Term Review Mission led by Dr. Tomohiko Sugishita, Senior Advisor, JICA Tokyo, visited Madhya Pradesh from 16th to 25th February 2009 to review the Project activities (January 2007 – December 2008). Ms. Yu Sasaki, JICA India Office was a part of the Mission. The team met the Health Commissioner and other State Health officials, and also had an informal meeting with the Development Partners in the State.

The Mid Term Review meeting was held on 23rd February 2009, at the NRHM building, Bhopal. The Mid Term Report prepared by the Project was released by the Health Commissioner. Dr. Sugishita made a brief presentation on the findings of the Project review and emphasized on the following:

  • Optimal achievements of the Project and lessons learnt within a short period

  • Recommendation to focus more on the activities for the ANM cadre and related functions

  • The Project is a typical case of Technical Cooperation

 

Following the review, the Minutes of the Meeting between the Japanese MTR Mission and the Department of Health & Family Welfare, Government of Madhya Pradesh were signed.

Mid Term Review Mission and the Project Formulation team visited the Project site.

The Mid Term Review mission and the Program Formulation Study Mission, led by
Dr. Tomohiko Sugishita, Senior Advisor, JICA Tokyo, visited Damoh district (Project intervention site) from 18th to 20th February 2009. Dr. Yoichi Yamagata, Project Manager; Ms. Mari Tsuda, Project Coordinator; Mr. S.Fareed Uddin, TQM Consultant; and Dr. Aboli Gore, HRM Consultant (JICA/MP-RHP team) accompanied the Mission. The idea was to introduce the Mission to the entire Health system at different levels.

On the way to Damoh district, the team visited non-intervened areas - SHCs Nunsar and Udna sadak of block Patan and CHC Patan in Jabalpur district and interacted with the health workers and community.

In Damoh district (JICA intervened area), the team first went to CHC Tendukheda, the pilot block, where they interacted with the BMO. The DPM was also present at the CHC. This was followed by the visit to PHC Sarra and SHC Taradehi. The DPM shared in detail about the changes that had occurred in Tendukheda block, as well as, in the entire district due to the Technical Assistance provided by JICA.

There was an evident difference between the non-intervened and the Project intervened areas.

Dr. S.K.Shrivastava, Joint Director, RCH/NRHM, joined the Mission in Damoh on
19th February 2009. The Mission shared the field observations with him.
Dr. Shrivastava then shared his observations about improvement in the quality of services after JICA interventions.

The team visited CHC Pathariya on the way to Sagar district, and interacted with the BMO. In Sagar, the team went to the District Hospital and saw the new Maternal and Child Wing and the proposed new teaching hospital. The team then met the faculty of the ANMTC and interacted with the participants. The CMHO, the Civil Surgeon, and the Divisional Program Manager were also present. The team also met the District Collector.

Formulation of State Core Groups

Dr. Manohar Agnani, Health Commissioner called a meeting of all the Development Partners, on 21st January 2009, to discuss the possibility of coordinated working in the State. The Partners included UNFPA, UNICEF, DfiD, JICA, IPAS, NIPI, PATH and MP TAST. Following this, he has formulated Thematic Core Groups at the State level with the Development Partners and officials of Departments of Health and Women and Child Development.
The 2 Core Groups formulated are: 1) Nutrition Core Group and 2) Equity, Access and Community Mobilisation. JICA is an active member of both the Core Groups.

Preparation of video films

On request from the State, the Project decided to prepare video films on VHND, ANC clinic and Maternity Wing organization. The film shooting was conducted in Damoh district from 24th to 26th February 2009. Ms. Rohini Jinsiwale (Training Coordinator) and Ms. Dimple Save (Documentation Coordinator) accompanied the crew to facilitate the film shooting.

3. Meetings

  • Ms. Dimple Save, Documentation Coordinator, JICA/MP-RHP met Dr. Veena Sinha, In charge IEC Bureau, on 19th February 2009, to share the tools and materials developed by the Project and explore the possibilities for wider replication.

  • Ms. Richa som, HMIS Consultant, JICA/MP-RHP, participated in the meeting on “Consultation meeting on issues/concerns of NRHM/RCH Program” on 18th February 2009. The panel included the Principal Secretary Health, Commissioner Health, Dr. Doke (Advisor to the Government of MP), Dr. Dixit and Dr. Saini.

  • Ms. Dimple Save, Documentation Coordinator, JICA/MP-RHP, participated in the dissemination workshop on “State Breastfeeding Trends Initiative” organized by Department of Pediatrics, G R Medical College, Gwalior, on 7th February 2009.

  • A Convergence meeting between the Departments of Health and Women & Child Development was initiated by the Health Commissioner, Dr. Manohar Agnani, on 2nd February 2009. The Principal Secretaries of both the Departments chaired the meeting. The participants included State officials of both the Departments as well as the Development Partners.

The participants were divided into 5 working groups namely,

1. Infant Feeding Practices

2. Complete Immunization

3. Ante-natal, Natal and Post-natal care, referral and Population Stabilization

4. Growth monitoring with special attention to Grade 3 and 4 children and

5. Operationalisation of Village Health and Sanitation Committee, Village Health and Nutrition Days and Swasthya Chaupal

Each group made a presentation on the action plan developed through the group exercise. Some key decisions were taken during the meeting, the final document for which is awaited.

JANUARY 2009

1. Summary

  • Ms. Yumiko TANAKA, Senior Advisor (Gender and Development), JICA, Tokyo, was on her second visit to Madhya Pradesh from 6th to 16th January 2009. In order to assess the impact of the Project activities on the empowerment of female staff, Ms. TANAKA visited health facilities at various levels in Tikamgarh district (District Hospital, CHC, PHC and SHC) and interacted with the staff.
    At the district level, Ms. TANAKA had interaction with the CMHO, Civil Surgeon, DPM, IEC Consultant and DPO WCD. She also met the District Collector,
    Mr. K.P.Rahi, who gave her a brief account of the gender programs in Tikamgarh district.
    Ms. TANAKA shared her field observations in the Workshops in Sagar and Bhopal.

  • In Tikamgarh district, 5 CHCs and 3 PHCs were visited to follow up on the project interventions.

Tikamgarh

  • JICA coordinators (Ms. Rohini Jinsiwale and Ms. Dimple Save) visited 5 CHCs (Badagaon, Baldevgarh, Khargapur, Jatara and Palera) and 3 PHCs (Sarkanpur, Bamorikala and Digoda) to facilitate re-organisation of maternity wings and establishment of new born corner, as well as, follow up of previous activities/interventions.

  • At CHC Palera, besides the delivery register, the maternity wing staff is maintaining records of initiation of breastfeeding, use of radiant warmer and immunization of the new born.

  • Partographs are being maintained regularly at 4 CHCs (Palera, Jatara, Baldevgarh and Khargapur) and at PHC Bamorikala.

Panna

Dr. Choudhary, BMO Amanganj of district Panna organized an orientation of the ANMs on use of BP instrument during the block meeting and 27th December 2008. JICA Block Coordinator (Mr. Maroti Lohat) also oriented them on organization of ANC clincs and VHNDs.

Gender mainstreaming workshops

A divisional workshop on “Gender Mainstreaming in NRHM Programs” was organized by JICA/MP RH Project in Sagar on 13th January 2009, in which the DPM, SN, ANMTC tutor, ANM and MPWm from Damoh and Tikamgarh districts shared their experiences of empowerment through JICA/MP-RH Project interventions. Ms. Yumiko TANAKA analysed these experiences according to the Gender Analysis Framework.

The participants included CMHO, Civil Surgeon, DPO WCD, DPM, IEC consultants and counselors of all the 5 districts of Sagar division. Dr. K.K.Tamrakar, Regional Joint Director, DoHFW, and Ms. Seema Thakur, Divisional Joint Director, WCD, were present during the entire workshop and gave their valuable inputs.

In the State workshop held in Bhopal on 15th January 2009, Dr. K.K.Tamrakar, Regional Joint Director, DoHFW first shared the findings of the divisional workshop. Ms. TANAKA further analysed these findings from the viewpoint of the opportunities for Gender Mainstreaming of RCH program. The participants included State level officials; NRHM consultants; and representatives from various Development Partners, among which UNICEF and UNFPA showed keen interest to collaborate with JICA and create workable models in the State. Dr. S.K.Shrivastava, Joint Director, RCH, DoHFW, gave the concluding remarks.

4. Meetings

  • JICA team (Dr. Yoichi Yamagata, Mr. S.Fareed Uddin, Dr. Aboli Gore and Ms. Richa Som) attended the meeting called by the Health Commissioner on 21st January 2009, to discuss with the NHSRC team about DHIS software being used in the State for NRHM and RCH II reporting. The other members included representatives from Dfid TAST and SPM Unit, NRHM.

  • The Health Commissioner called for a meeting of all the Development Partners on 21st January 2009, to discuss the possibility of coordinated working in the State. The Partners included UNICEF, DfiD, JICA, IPAS, NIPI, PATH and MP TAST.

5. Visitors

  • Ms. Yumiko TANAKA, Senior Advisor (Gender and Development), JICA, Tokyo, was on her second visit to Madhya Pradesh from 6th to 16th January 2009. In order to assess the impact of the Project activities on the empowerment of female staff, Ms. TANAKA visited health facilities at various levels in Tikamgarh district (District Hospital, CHC, PHC and SHC) and interacted with the staff.
    At the district level, Ms. TANAKA had interaction with the CMHO, Civil Surgeon,
    DPM, IEC Consultant and DPO WCD. She also met the District Collector,
    Mr. K.P.Rahi, who gave her a brief account of the gender programs in Tikamgarh
    district.
    Ms. TANAKA shared her field observations in the Workshops in Sagar and Bhopal.
    Her main message was that many female staff including ANMs, LHVs and LMOs
    have been empowered in the full spectrum of the five aspects of empowerment –
    Psychological, Physical, Economical, Social and Political.

  • Mr. Yushi Nagano, Representative (OJT) JICA India office, visited Madhya Pradesh from 15th to 16th January 2009 to observe the Project activities.

1. Summary

  • JICA/MP-RH Project intervention districts i.e. Damoh and Tikamgarh have been identified for initiating Maternal and Perinatal Death Inquiry and Response (MAPEDIR) activity. A TOT (Training of Trainers) was organized by UNICEF from 26th – 29th December 2008.
    Mr. K.P.Rahi, who gave her a brief account of the gender programs in Tikamgarh district.
    Ms. TANAKA shared her field observations in the Workshops in Sagar and Bhopal.

Damoh

  • JICA team facilitated distribution of materials (Radiant Warmer, labour table, medicines etc.) to the 3 PHCs (Raneh, Hinota and Madiyado) from CHC Hatta and oriented the staff of PHC Hinota on organization of the maternity wing.

  • Twenty-one SHCs (3 per block) were identified by the district in June 2008 for upgrading them to provide 24x7 delivery services. JICA training coordinator

  • (Ms. Rohini Jinsiwale) had oriented them on organization of labour room. During the visit to Damoh this month, JICA team assessed SHCs Raja Patna and Lakhlakha of block Patera which have been identified for 24x7 delivery services and gave the necessary suggestions to the health workers.

  • At CHC Patera, Partograph is being maintained for each delivery case. The records of the labour room, including information about the birth weight and initiation of breastfeeding within one hour are being regularly filled.

  • JICA team has started facilitation at PHC Abhana of block Hindoriya for re-organisation of the entire facility.

  • JICA team comprising of Dr. Yoichi Yamagata, Project Manager; and Ms. Rohini Jinsiwale had discussion with Dr. Ahirwar, BMO Hindoriya about the re-organisation of the maternity wing. Dr. Bajaj, District Training Officer accompanied JICA team to Hindoriya.

  • JICA Training Coordinator (Ms. Rohini Jinsiwale) oriented the ANMs of Hindoriya block on ANC clinic organization.

  • Mr. Jaimon Thomas, DPM and JICA Training Coordinator (Ms. Rohini Jinsiwale visited CHC Hatta and PHC Hinota. The DPM gave instructions to shift the materials stored in the eye hospital building to the store and to convert it into a ward at CHC Hatta; while at PHC Hinota, he instructed the staff to set up the labour room and re-organise the store in one single room.

Tikamgarh

  • JICA coordinator oriented the LR staff of CHC Baldevgarh on further improvement in the maternity wing, including recording in registers as well as maintaining handing over register.

  • Orientation of LR staff on re-organisation of maternity wing at CHC Khargapur.

  • JICA coordinator also visited CHCs Palera, Jatara and Prithvipur for follow up of the suggestions given during previous visits. Maternity wing at CHCs Jatara and Palera were well maintained. However, at CHC Prithvipur, the maternity wing was disorganized and requires proper management for improvement.

  • JICA coordinator attended the block meeting at CHC Niwari and oriented the staff on re-organisation of SHCs and organization of VHNDs.

New Districts

  • Dr. Choudhary, BMO Amanganj of district Panna organized an orientation of the ANMs on use of BP instrument during the block meeting and 27th December 2008. JICA Block Coordinator (Mr. Maroti Lohat) also oriented them on organization of ANC clincs and VHNDs.

MAPEDIR

  • The National review of Maternal and Perinatal Death Inquiry and Response (MAPEDIR) for six States of India was organized by UNICEF on 7th October 2008, New Delhi. UNICEF MP is planning on scaling up this activity across the State in partnership with other development partners like JICA. To begin with, a TOT on MAPEDIR was organized in Damoh district. JICA/MP-RH Project team supported the district in planning and organization of the workshop.
    In all, 18 participants covering all the 7 blocks were trained on investigation of
    maternal deaths.
    Ms. Rohini Jinsiwale (Training Coordinator) and Ms. Dimple Save (Documentation
    Coordinator) from JICA/MP-RHP attended the training.

4. Meetings

  • Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP attended the meeting of Development Partners held on 4th December 2008 in Bhopal, to review the areas of technical assistance provided by each partner and to explore the possibilities of convergent activities in any one district of the State.

  • Mr. S.Fareed Uddin, Project Operations Manager and TQM Consultant and
    Dr. Aboli Gore HRM consultant met Ms. Alka Upadhyay, Health Commissioner to discuss about the areas where JICA project could provide technical assistance, like Core competency training for ANMs on antenatal care, NRHM reporting formats, sharing good practices for operationalisation of VHNDs.

1. Summary

  • Upgradation of SHCs has been initiated in 17 SHCs of Chhatarpur and Panna districts.

  • The Project has gone one step further and initiated upgradation of the PHCs. In the month of November 2008, 7 PHCs of Tikamgarh district were taken up.

  • The Project is in the process of preparing for the Mid Term Review by JICA Mission in mid-February 2009. A meeting was held with the field coordinators of the project on 26th November 2008, to orient them on data collection for the review.

Damoh

  • Maternity wing organization at the District Hospital was in progress. JICA team facilitated issue of beds and other supplies for the observation corner as well as labour room.

  • Labour room set up at CHCs Pathariya and Tendukheda were maintained as suggested in previous month. JICA team oriented the LR staff of both the PHCs on filling up the Obstetric card and plotting of Partograph.

  • Facilitated issue of normal delivery kits from district store to CHC Tendukheda and PHC Sarra.

  • JICA team visited 5 SHCs to observe the performance of the staff. The SHCs were well organized and all the ANMs had started filling the MH cards.

Tikamgarh

  • JICA team visited 5 out of the 6 CHCs in the district. The labour rooms at CHCs Niwari, Palera, Jatara and Prithvipur were maintained as suggested during previous visit. However, the labour room at CHC Baldevgarh was in very poor condition.

  • JICA team facilitated cleaning and re-organisation of the labour room at CHC Baldevgarh, as well as, coordinated with the store-keeper to provide the necessary supplies.

  • JICA team visited 7 PHCs of the district, covering Niwari, Jatara, Palera and Baldevgarh blocks. Maternity wing is poorly maintained at PHCs Teherka (Niwari), PHC Lidhora and Digoda (Jatara), PHC Sarkanpur and Khargapur (Baldevgarh) and needs re-organisation.

  • The MO, Dr. Akhilesh Ratnakar, of PHC Baranathar of block Palera has taken initiative to provide baby wrap for each new born delivered at the PHC.

  • JICA team facilitated re-organisation of the labour room at PHC Bamhorikala of block Palera.

  • District Hospital – A visible change was observed in the PNC wards. The ward was very clean and organized and sheets were available on all the beds. The Civil Surgeon, Dr. Sunit Jain, is taking a lot of interest for improving the services in the maternity wing. However, there is a shortage of human resource at the DH.

New Districts

  • Upgradation of SHCs has been initiated in 8 SHCs of Nowgaon block of Chhatarpur
    district and in 9 SHCs of Amanganj block of Panna district.

4. Meetings

  • Ms. Mari Tsuda, Project Coordinator, JICA/MP-RHP presented her observations on 24th November 2008, between:

    • the project’s approach

    • Morogoro Health Project in Tanzania and

    • conventional JICA project

  • Ms. Mihiko Tamamura, Country Program Director, WFP, accompanied by Ms. Usha Goyal, State Director, WFP MP and Mr. Anand, WFP MP, visited JICA/MP-RH Project office on 17th November 2008. Ms. Tamamura informed that ‘Food for Human Development’ was the new modality of their program and she would like to promote the same in India. This could offer a good opportunity for convergence. She also showed interest in the operational manual on ‘Gaud Bharai’ as well as the other tools developed by the project.

  • The Ministry of Health & Family Welfare, Government of India and WHO India had organized the “Expert Group Meeting for review of SBA Training Package” on
    7th November 2008. The meeting was coordinated by CEDPA, New Delhi.
    The purpose of the meeting was to review and revise the existing training package of SN/ANMs and LHVs, to make it more comprehensive and user-friendly.
    Dr. Aboli Gore, HRM Consultant and Mr. S.Fareed Uddin, Project Operations Manager & TQM Consultant represented JICA/MP Reproductive Health Project at the meeting. The made a presentation on the current status of SBA training in Madhya Pradesh and recommendations/suggestions for further improvement.

  • Ms. Usha Goyal, State Director, WFP MP, visited JICA/MP-RH Project office on 5th November 2008, to explore the possibilities of coordination between WFP and JICA/MP-RHP. Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP suggested that it would be useful for both the agencies to work jointly in the fields of Tikamgarh and Chhatarpur districts.

Maternity wing organization

  • Facilitation of maternity wing organization continued in the district hospitals of Damoh and Tikamgarh districts.

  • Re-organisation of maternity wing and setting up of ANC clinics at CHCs Hatta and Tendukheda, and PHC Sarra of district Damoh and CHCs Niwari, Prithvipur, Palera and Jatara of district Tikamgarh.

  • Three months stock of MH cards (14,000) was supplied to Damoh and Tikamgarh districts.

  • JICA team visited five SHCs of Damoh and three SHCs of Tikamgarh districts. The overall organization of the SHCs was satisfactory. The team gave few relevant suggestions for each centre.

4. Meetings

  • JICA team (Mr. S.Fareed Uddin, Project Operations Manager and Dr. Aboli Gore, HRM Consultant) had a meeting on 16th October 2008with Dr. Hamid El Bashir, State Representative, UNICEF, and Mr. Taposh Roy, DfID, to extend invitation for the proposed field visit to JICA/MP-RHP site. Discussions were also held with Dr. Hamid about UNICEF-JICA project partnership for MAPEDIR in Damoh and Tikamgarh districts.

  • Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP, met Mr. Irigaki, the new Resident Representative of JICA on 14th October 2008. Mr. Irigaki appreciated the project efforts and showed his interest in having the JOCV volunteers in Madhya Pradesh.

  • Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP met Mr. Sanjay Prasad at Nirman Bhavan, New Delhi, on 14th October 2008 and explained to him about the plan of TQM training to be held in Sri Lanka and extended the invitation for few officials from the MoHFW, Government of India, to participate in the training. Mr. Prasad showed keen interest and has requested for more details regarding the training, so that he could depute the relevant officials for the same.

  • On 13th October 2008, Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP visited NHSRC in their new premises in NIHFW campus. He met Dr. Sundararaman and his team. They informed that:

    • JICA’s contribution and collaboration was acknowledged during the Governing Board meeting held on the same day.

    • Dr. Sundararaman suggested that JICA should have a MoU with NHSRC for further collaboration.

    • Dr. Sundararaman also shared a draft of the Public Health Bill and its attachment, which suggests that in near future, SHRC, SPMU as well as SIHFW should be reinforced.

    2nd - 4th October 2008, at Calcutta. The theme of the conference was, “Healthy Mothers – Healthy Babies, Midwives continue care”. The Society of Midwives, India, invited JICA/MP-RHP to participate in the conference and share the project experiences. Dr. Aboli Gore, HRM Consultant participated in the conference and presented a paper on “Quality Innovations in Teaching Methodology”, wherein the experiences of JICA/MP-RHP on SBA training were shared. The participants included Principals of Nursing Colleges, Nursing Tutors and Nursing students from various parts of the country.

1. Summary

  • Field works were focused to the original pilot districts of Damoh and Tikamgarh.

  • Both the district hospitals and 13 CHCs were visited. New born corners have been established in 9 CHCs.

  • Maternal Health (MH) Cards have been reintroduced in Damoh and Tikamgarh. This is a revised version of the ANC cards, to which information on INC, PNC and NBC have been added after discussion with the Directorate of Health and Family Welfare, Bhopal.

  • JICA played an important role of customizing the Recording/Reporting formats introduced by Government of India through NHSRC (National Health Systems Resource Centre), New Delhi.

  • In order to accelerate implementation of the new format, JICA facilitated the identification of the source of data/information in the records/registers.

  • Village Health and Nutrition Days (VHNDs) have been performed in many villages in the past 3 months, so that JICA is shifting from the trial phase to the phase for comparison and standardization.

3.1 HRM
Maternal Health Card Orientation

  • JICA Consultants conducted orientation of health staff on Maternal Health Card in all the
    7 blocks of Damoh as well as 6 blocks of Tikamgarh.

  • The participants comprised of ANMs, MPWm, LHVs, MPS and BEE. In all 327 participants from Damoh and 224 from Tikamgarh districts were oriented.

Maternity wing organization

  • Facilitation of maternity wing organization has been initiation in both the district hospitals as well as in 11 CHCs and 5 PHCs of Damoh and Tikamgarh districts.

  • JICA Coordinators (Ms. Mari Tsuda and Ms. Rohini Jinsiwale) observed the VHND in village Ajnor of block Badagaon on 23rd September 2008. They were accompanied by JICA Block Coordinator, Mr. Priyank Dubey.

  • Dr. Yoichi Yamagata, Project Manager, JICA/MP-RHP and Ms. Dimple Save, Documentation Coordinator, observed the VHND in village Dumduma of block Prithvipur on 23rd September 2008. The coordination among the service providers (health and ICDS staff) was very good and thus enabled smooth delivery of services. JICA Block Coordinator, Mr. Manoj Chouhan, was also present.

4. Meetings

1) Meeting held for finalization of the NRHM reporting formats on 1st October 2008. This was the continuation of the discussions held on 26th September 2008. The meeting was held at the new NRHM building and was attended by the State officials, DFID TAST members and JICA consultants.
The State officials have JICA requested to design MTP register also.
It was decided that the reporting / recording formats shall be shared with the districts during divisional Maternal Health review (starting on 4th), where JICA would be providing technical assistance.
The SDOs are planning to orient the districts instructing them to start the process of using these formats (starting from 13th), where JICA would be involved in the orientation.

2) Discussion held on matching the recording formats with new NRHM/RCH formats shared by MoH&FW, New Delhi, on 26th September 2008. The participants included Dr. Jayshree Chandra, Deputy Director (Child Health); Dr. Archana Mishra, Maternal Health Consultant;
Dr. Ashwini Bhagwat, State Immunization Officer; and Mr. Neeraj Shrivastava, SDO SPMU from the DoHFW and JICA consultants.

3) Meeting called by Mr. Ashish Choubey, State IEC Consultant, NRHM/RCH, on
17th September 2008, for finalizing the film on JSY promotion. The participants included
Dr Jayshree Chandra, Deputy Director (CH); Dr. Rajan Dubey, NIPI; NRHM Consultants –
Mr. SatishShrivastava,Ms.Mangalika Tripathi, Ms. Shailey ; Dr. Rajiv Shrivastavs – JSY Coordinator; Mr. DubeyIECBureaustaff; Mr. Bhai Shelley, UNICEF; and JICA Cooridinators
(Mr. Arshad Khan and Ms. Dimple Save). The film producers were also present. The participants saw the film and gave their inputs for revision.

4) Dr. Yamagata Yoichi, Project Manager; and Ms. Mari Tsuda, Project Coordinator, JICA/MP-RH Project visited New Delhi to attend the farewell party for Mr. Fuji on 10th September 2008. They also met Mr. Fukuda E.J., JBIC staff, prior to merger of JBIC into JICA on 1st October 2008.

5) A divisional review meeting was called by the RJD of Sagar division on 5th September 2008. The meeting was chaired by Dr.K.K.Tamrakar, RJD. The participants included CMHOs and DPMs of all 5 districts of Sagar division. JICA team comprised of Dr. Yoichi Yamagata, Project Manager; Mr. Fareed Uddin, TQM Consultant; Dr. Aboli Gore, HRM Consultant; and Mr. Arif Anees, Block Coordinator, Sagar.
JICA team shared the project interventions with the entire team.
Dr. K.K.Tamrakar, Regional Joint Director, Sagar division, officially announced the initiation of JICA project’s intervention in Panna and Chhatarpur districts.

Visitors

Ms. Ai Yamamoto, an intern, was sent by JICA India office to visit JICA MP Reproductive Health Project site from 23rd – 26th September 2008. She visited Tikamgarh district along
with the project team to study the field situation.

  • The institution has started conducting deliveries on delivery tables.

  • Labour room is organized, but continuous supply of materials and service delivery needs to be ensured.

  • Colour coded waste bins are being used in the labour room.

  • CHC Tendukheda

  • Cleanliness is being maintained in the labour room.

  • Oxygen cylinder has been shifted to the labour room, but it is not functional. JICA Coordinator has asked the staff nurse to follow up the repair of oxygen cylinder.

  • Partograph is being filled only after the woman is admitted into the labour room.

CHC Patera

  • The labour room is being maintained well. Construction of the new labour room has not yet started.

  • The staff nurse, Ms. Mubina, is maintaining the records properly. All the case sheets have partographs enclosed (properly filled).

  • She is also maintaining the record of the new born who have been breastfed within one hour of birth as well as their birth weight.

  • She informed that she had issued 25 draw sheets for the beds and they were using them.

  • She also informed that she was getting good support from the BMO.

Tikamgarh
PHC Orchha

  • Labour room is very small, but well organized.

  • Workers are following infection prevention procedure in the facility by keeping the instruments in savlon solution.

  • JICA Coordinator shared the simplified copy of partograph with the staff and oriented them on filling it.

CHC Niwari

  • Labour room is partially organized.

  • JICA Coordinator (Mr. Arshad Khan) facilitated the VHND in village Devri Ratan of block Patera on 26th August 2008. JICA Coordinator (Ms. Dimple Save) observed and documented the entire activity

Visit to Panna and Chhatarpur

JICA team visited the districts to initiate the project interventions.

Chhatarpur
District headquarter

  • Meeting held with CMHO, Civil Surgeon, RMO, DPM and the nodal officer for SBA training and management to discuss about the project interventions.

  • Discussions were held on planning and organization of SBA training.

  • The team visited the labour room at the District Hospital. JICA team shared the illustrated guidelines and gave suggestions for organization of the maternity wing.

CHC Nowgong

  • The labour room is quite small, while the delivery load is 250 per month.

  • JICA team suggested the BMO to convert the newly constructed staff duty room into the labour room for better service provision.

  • Details of BMW authorization were shared with the BMO and space for sharp and deep burial pits was identified.

CHC Rajnagar

  • This was the second visit to the facility by JICA team.

  • The facility layout has been painted on the outer wall of the CHC.

  • Nutrition Rehabilitation Centre is well established.

  • Female sweepers have been identified for the labour room.

  • JICA team gave suggestions for organization of the labour room and shared the illustrated guidelines.

  • JICA HMIS consultant shared the revised form 6 with the DPM to be shared with the health workers.

Panna

  • JICA team met the DPM, Mr. Maharaj Singh, and explained the project activities in brief.

  • JICA team shared the following information/documents with the DPM:

    • 12 steps to organize maternity wing and illustrated guidelines

    • Operationalisation of Village Health and Nutrition Days (VHNDs)

    • Revised form 6 to be filled by the health workers

    • Specifications for BMW management and details about sharp and deep burial pits

  • JICA team visited the district hospital. The building is old, but kept clean.

  • Separate new born care unit has been established.

  • New female and pediatric wards as well OT are under construction

  • Proposal has been sent for construction of new labour room

CHC Amanganj

  • JICA team, accompanied by the DPM, visited the CHC.

  • It was the day of block meeting and JICA team got the opportunity to interact with the field staff.

  • Microplans are available only for immunization and not for ANC clinics

  • 5 sweepers have been appointed, however cleanliness is not being maintained.

  • Nutrition Rehabilitation Centre is well established.

  • JICA team gave suggestions for organization of the maternity wing as well as re-organisation of the admin block.

4. Meetings

  • Meeting was held at the Department of Health & Family Welfare on 27th August 2008, to arrive at a consensus on Maternal and Child Health Cards to be used in the State. The participants included State NRHM Consultatns, members from UNFPA, UNICEF, DfiD TAST and JICA. The meeting was chaired by Dr. K.K.Shukla, Director, Health & Family Welfare.

  • Dr. Yoichi Yamagata, Project Manager, JICA/MP-RH Project, delivered a lecture on “Experience of Health Development in Japan” to Post-graduate students (40) at IIHMR, Jaipur on 26th August 2008.

  • A preparatory meeting was organized by the State IEC Consultant, Mr. Ashish Choubey, on 7th August 2008, to finalise the State IEC/BCC strategy. The participants included representatives from State IEC Bureau, UNICEF, UNFPA, DfiD and JICA.

5. Visitors

  • Prof. S. Watanabe, Mr. Hirau, Administration Assistant and Mr. Yoshioka Kota, post-graduate student from Nagasaki University visited Madhya Pradesh from 12-14 August 2008. The purpose of the visit was to prepare for the internship of Mr. Kota in India, specifically in JICA/MP-RH Project.
    They also visited Damoh district to observe the project interventions and held discussion with Regional Joint Director, Dr. K.K.Tamrakar, at RJD office, Sagar.

1. Summary

  • In the month of July 2008, 8 Village Health & Nutrition Days (VHNDs) were organized in the Project area.

  • A three days orientation workshop on Training Methodology for improving the quality of SBA training was conducted by the Project for Sagar division from 22nd to 24th July 2008. Dr. Bhushan, Assistant Commissioner, Maternal Health, MoHFW, Government of India, visited the orientation workshop, accompanied by Dr. Archana Mishra, MCH consultant, GoMP.

3.1 HRM
SBA TOT, Bhopal

  • A three days orientation was organized by JICA/MP-RH Project from 22nd – 24th July 2008 in Bhopal.

  • The purpose of the workshop was to orient the participants on the training methodology for improving the quality of SBA training.

  • In all, 17 participants covering all the 5 districts of Sagar division attended the orientation. The participants comprised of Civil Surgeon, CMHO, Medical Officers, District Training Officer, DPHNO, Principal and Tutors of ANMTC. Apart from these, one of the Child Survival Coordinator from UNICEF Patna also attended the orientation.

  • The facilitators guide and teaching aids developed by JICA/MP-RHP were used during the orientation, apart from the set of 3 modules for SBA training (ANC, INC, PNC).

  • The teaching aids were highly appreciated by the participants.

  • Dr. Bhushan, Assistant Commissioner, Maternal Health, MoHFW, Government of India, visited the orientation workshop, accompanied by Dr. Archana Mishra, MCH consultant, GoMP.

  • JICA/MP-RHP team showed him the Facilitator guide (draft) and other teaching aids developed for the orientation. He expressed his happiness that all the tools and materials were developed in line with the GoI guidelines. Dr. Archana Mishra also confirmed that all the materials were developed by JICA/MP-RHP after taking prior approval from the State authorities. He was very thrilled to see the materials and commented, “JICA has done lot of good work silently, without taking any claim.” He has asked JICA team to send a complete package of the training materials to Delhi, so that he can show it to his colleagues.

  • Dr. Bhushan showed a lot of interest in the training methodology and interacted with the participants of Damoh, Tikamgarh, and Chhatarpur on the training methodology, use of teaching aids and time spent for using these aids. The participants gave very positive feedback and expressed their satisfaction.

  • Dr. Bhushan appreciated the efforts of JICA/MP-RHP and asked if the team could also develop some video films on topics like normal delivery, management of PPH, convulsions, eclampsia, stages of labour etc. He emphasized that video films should be the prime agenda for all the trainings.

Meeting with CMHO, Damoh

  • JICA Training Coordinator shared the observations of the labour room at the district hospital with the CMHO, Dr. Jain.

  • The CMHO has agreed upon the following tasks to be completed for the labour room:

    • Removal of fixed cemented platform type labour table

    • White washing of labour room

    • Establishment of new born corner in labour room and shifting the equipments

    • Provision of almirah in labour room for storing instruments and other materials

    • To place one person as in charge of the maternity wing

    • Converting private room into observation room

3.2 TQM

CHC Patera

  • JICA Training Coordinator attended the block meeting on 30th June 2008.

  • In all 26 out of 34 health workers attended the meeting.

  • On request of the BMO, Dr. Prahlad Patel, JICA Coordinator oriented the health workers and supervisors on Microplanning exercise.

  • At the end of the session, microplans with maps were prepared by all the health workers.

  • The BMO instructed the entire staff to start working according to the microplans prepared and to display the map and tour plan on the SHCs.

3.3 HMIS
Validation exercise at CHC Pathariya

  • JICA Training Coordinator oriented the supervisors on validation of data reported by the health workers.

  • The BEE and Computer also attended the half day orientation.

  • The supervisors were asked to do validation of any 3 SHCs of their area and the formula for calculation of variation was also shared with them.

  • At the end of the session, the supervisors had prepared their field plan for validation.

3.4 I
Village Health & Nutrition Day

  • The State officials had discussions with JICA/MP-RH Project team in the first week of July 2008, on operationalization of Village Health & Nutrition Days. The State officials have asked JICA team to initiate organization of VHND in their project areas and create a doable model for further replication in the State.

  • In the month of July 2008, a total of 8 VHND sessions were organized in the JICA/MP-RH Project intervention areas as below:

  • Ms. Sushila Thakur, AWW - “Earlier we had to go to every single house and call the children and women. But today because of this program, they started coming on their own”.

  • Ms. Kalpana Rajput, AWW Dinari – “This program is very good. Next Tuesday we will hold it in Dinari”.

  • Ms. Baribai Dhurve, Mahila Coordinator, Tendukheda - “This program has given an opportunity to the women to speak. Whatever they could not say at home, they could say it over here. Due to this program, all the activities have been completed systematically in a single day”.

  • Ms. Anita Thakur, ANM - “This program has really reduced our work. Earlier we had to go house to house to provide the services. But today, we could complete the work in about 4 hours. We also got lot of support from AWW, ASHA and JSR”.

1. Summary

  • An Orientation on Training Methodology for SBA training was jointly organized by the DoHFW, GoMP and JICA/MP-RHP at the State Institute of Health Management & Communication (SIHMC) Gwalior from 5th – 7th June 2008.

  • Two counterparts from State government - Dr. Shashi Thakur, Gynecologist, Sagar district; and Dr. Tulsa Thakur, Block Medical Officer, Damoh district participated in the regional workshop on “Sharing Experiences and Learning from Good/Successful Practices in the Islamic Communities” from 15th – 19th June 2008 at Aqaba, Jordon.

  • District Health Society meeting was held on 26th June 2008 at Damoh.

  • Staff Nurses, LHVs and ANMs of 5 out of 7 blocks of Damoh were oriented on Maternity Wing organization. While, ANMs (18) of 17 out of 21 SHCs (3 in each block) identified for 24 hour delivery services were oriented on labour room organization

  • Technical assistance provided for Layout of the new maternity wing under construction in District Hospital campus, as well as, for the new maternity wing at Civil Hospital Bina, Sagar.

3.1 HRM
SBA TOT, Gwalior

  • An Orientation on Training Methodology for SBA training was jointly organized by the DoHFW, GoMP and JICA/MP-RHP at the State Institute of Health Management & Communication (SIHMC) Gwalior from 5th – 7th June 2008.

  • The State was facing scarcity of human resource as tutors or facilitators. Hence, head hunting for retired nursing staff was initiated by JICA/MP-RHP after consultation with the State officials (DoHFW). An orientation for these identified persons was organized by JICA/MP-RH Project in collaboration with SIHMC (State Institute of Health, Management & Communication), Gwalior, from 5-7 June 2008. The training was conducted by JICA MP/RH Project in collaboration with the SIHMC. Apart from the set of 3 modules being used for the SBA (ANC, INC, PNC) training, a facilitator’s guide and teaching aids developed by JICA/MP-RHP were introduced.

  • The role of SIHMC has now been recognized officially as the Apex Training Institute, as it functions as the direct link between the State and the RHFWTCs (Regional Health & Family Welfare Training Centres).

  • In all 33 participants comprising of Medical Officers, Retired as well as In-service Staff Nurses and LHVs, DPHNs, Sister Tutors and Counsellors, covering 12 districts attended the training. It is worth noting that participants also included representatives from all the
    3 RHFWTCs (Regional Health & Family Welfare Training Centres) in the State, namely, Gwalior, Indore and Jabalpur. The involvement of the RHFWTCs is very crucial, as besides skill enhancement, the training covered issues like planning, management and monitoring, which could be taken forward by the RHFWTCs.

  • A state wise scale up plans for SBA TOT and IMEP training were drafted jointly by faculties of SIHMC, all 3 RHFWTCs and JICA team. The diagrammatic Plan of Action is enclosed herewith (Annexures I and II).

Regional Workshop, JORDON

  • A regional workshop on “Sharing Experiences and Learning from Good/Successful Practices in the Islamic Communities” was organized by JICA, Jordon Reproductive Health Project from 15th – 19th June 2008 at Aqaba. The purpose of the workshop was to share good practices and successful models for replication in other countries.

  • In all 31 participants from 6 countries namely Palestine (6), Syria (3), Sudan
    (4), Afghanistan (5), Jordon (10) and India (3) attended the workshop. Country specific presentations were made by each team followed by the skill building sessions.
    JICA/MP-RH Project team comprised of Dr. Aboli Gore, HRM Consultant, JICA; Dr. Shashi Thakur, Gynecologist, Sagar district; and Dr. Tulsa Thakur, Block Medical Officer, Damoh district.

  • The skill building session by JICA/MP-RH Project was on ‘Capacity building of health providers as managers’, wherein, case studies on Auxillary Nurse Midwife (ANM), Block Medical Officer (BMO) and District Program Manager (DPM) were shared with participants during the group exercise. The groups discussed on the good practices observed in each case study and mechanism to sustain these practices.

  • The tools and materials developed by JICA/MP-RH Project were highly appreciated during the display at the stalls and participants from other countries requested for copies of the materials.

  • The details are given in Annexure A – F.

Damoh

  • JICA Training Coordinator (Ms. Rohini Jinsiwale) oriented SN, LHV and ANMs of 5 blocks on maternity wing organization on 17th June 2008. In all 18 participants attended the orientation program.

  • ANMs (18) of 17 out of 21 SHCs (3 in each block) identified for 24 hour delivery services were oriented on labour room organization on 18th June 2008. The SHCs have been identified on the following criteria:

    • Performance of workers

    • Availability of government/rented building

    • Worker staying at headquarter

  • Mr. Jaimon Thomas, District Program Manager, was also present throughout the orientation. JICA Coordinator, accompanied by the DPM and Ms. Jasmin Ram, SBA training in-charge, visited SHC Basa which has been identified for 24 hour delivery.

  • JICA Coordinator explained the ANM about labour room organization and shared a sketch of appropriate layout. The DPM also informed that he would soon be releasing the fund of Rs 25,000, allotted for the SHCs, under the head of renovation and repairs.

  • The ANMs and LHVs of CHC Patera have started filling the newly introduced MH cards. However, they have not yet started filling the Obstetric card.

  • There is no stock of TT vaccine at the district store as well at the blocks. The CMHO has asked the ANMs to purchase the vaccine for their respective centres using the untied funds.

3.2 TQM
Sagar
Duffrin Hospital

  • The Civil Surgeon requested JICA to provide technical assistance in planning the layout of the maternity wing in the new building.

  • JICA TQM Consultant (Mr.S.Fareed Uddin) had a meeting with Dr. P.K.Mishra, Civil Surgeon; Dr. Shashi Thakur, Gynecologist and Dr. Abhishek Verma on the layout plan on 25th June 2008.

  • The team visited the construction site, where, JICA consultant gave several suggestions. The suggestions were well taken and the construction supervisor asked Dr. Verma to provide an official letter to the Executive Engineer, PWD, with the suggestions.

Bina Civil Hospital

  • The Civil Hospital at Bina is a 60 bedded hospital catering to a population of 80,000. The delivery load on an average is about 200 per month whereas the OPD is approximately 250 per day. Dr. M.C.Meghwani, I/C Medical Officer at Bina Civil Hospital, Sagar district requested JICA/MP-RH Project team to provide technical assistance for the layout of the new maternity wing being constructed. The financial support has been provided by Bina Refinery, Bina.

  • JICA project team held consultation with the Bina Civil Hospital team comprising of
    Dr. M.C.Meghwani, Dr. (Mrs.) Parihar (Gynecologist), Dr. Sadhna Ahirwar (PGMO) and
    Mr. Shukla, PWD Civil Engineer on 16th June 2008.

  • A logical layout for the maternity wing including the labour room, 15-bedded female ward and staff duty room was proposed by JICA project team, which was highly appreciated by the doctors. Discussion was also held on arrangements for Bio Medical Waste management in the hospital campus.

  • The Civil Engineer, Mr. Shukla, agreed to change the layout design as per the suggestions given by JICA team and send it to JICA office for finalization.

 

Regional Office, Pollution Control Board

  • JICA Consultant met the Research Officers and shared the posters and stickers developed by the project

Damoh
District Health Society meeting

  • JICA TQM Consultant, Mr.S.Fareed Uddin attended the DHS meeting held in Damoh on
    26th June 2008.

  • The meeting was chaired by the District Collector, Mr. R.A.Khandelwal.

  • The participants included officials of Health, PWD and RES departments.

  • The officials confirmed that 15 SHC buildings will be handed over to the health department by 15 July 2008 and the remaining 9 buildings by the end of August 2008.

  • The second set of 15 SHCs under construction will be handed over by the end of August 2008.

  • JICA Consultant made a presentation on the Project components and activities in the district and shared a copy of the Illustrated Guidelines for the SHCs, developed by the Project.

  • The cooperation of the district and block officials (CMHO, DPM, BMOs Hatta, Tendukheda and Pathariya) were duly acknowledged.

Comments by the officials:

  • CM&HO, Dr. Ashok Jain said that after JICA Project’s training to the field ANMs, the women referrals have become very organized and only those women who need actual medical care come to the DH.

  • BMO, Hatta, Dr. A. Shrivastava acknowledged that the Quality Circle meetings have induced a feeling of responsibility amongst the workers and the acknowledged that tree plantation done in the CHC was the result of QC meeting facilitated by JICA Consultants.

  • BMO, Pathariya, Dr. E. Minj acknowledged the JICA Project’s consultation for maternity wing organisation.

  • BMO, Tendukheda, Dr. (Mrs.) Tulsa Thakur acknowledged the JICA Project consultation on SHC organisation.

 

PHC Sarra

  • DPM accompanied JICA team to PHC Sarra on 27th June 2008.

  • The windows of LR have frame and glass panes put up. However, the ventilator needs to be redone.

  • DPM has asked Dr. Namdev to visit the PHC thrice a week. He also instructed RCH accountant, Mr Vishal, to visit along with Dr Namdev, while Pharmacist Mr Meshram to visit once a week along with Dr Namdev.

  • DPM instructed the ANMs to charge Rs 2/- as RKS fee for registration and not Rs 5/-, and to maintain a register for the same.

  • DPM has asked Dr Namdev to open the bank account and take charge of JSY payments, and instructed the ANMs to maintain the JSY registers properly.

  • Block meeting was held on 27th June 2008 and for the first time an agenda was prepared for the meeting.

  • The BMO, Dr. Tulsa Thakur commented – ‘Earlier we didn’t know what to do in the block meeting. But, this time, as the agenda was prepared in advance, the meeting was very fruitful. Now onwards, no meeting will be held without any agenda’.

  • JICA team comprising of Mr. S.Fareed Uddin and Mr. Arshad Khan, were also present during the meeting.

  • Discussion on operationalising VHND by clubbing it with VHSC was discussed. JICA IEC Coordinator facilitated the discussion. Ten SHCs have been identified for this.

  • Villages for operationalising VHND have been identified from SHCs Tejgarh, Harrai, Dinari, Taradehi and Sailwara.

  • DPM instructed the ANMs to use their untied funds properly and timely and informed about the audit in the end of July 2008.

CHC Hatta

  • JICA IEC Coordinator (Mr Arshad Khan) shared the format developed on stock and distribution of IEC materials with the DPM and IEC Consultant, Ms. Neha.

  • He visited CHC Hatta on 17th June 2008 and oriented the BEE on the use of formats.

  • The IEC coordinator attended the video conference on ‘Orientation on VHSC’ on 19th June 2008, organised by the Directorate.

  • Proper guideline on utilization of funds by the VHSC is required.

 

4. Meetings

  • Meeting with Mr, Kailash Rai, CDPO, Tendukheda block, Damoh on 25th June 2008 at JICA/MP-RH Project office, Bhopal. The purpose of the meeting was to discuss on the implementation of VHND in some Anganwadi centres of Tendukheda block and develop models.

1. Summary

  • Three important events took place in May 2008 as follows:

    • TMH card orientation was held in Damoh and Tikamgarh involving 10 blocks out of 13 blocks. The participants included Medical Officers, BEEs, DEOs, health supervisors (M+F) and health workers (M+F). Each data entry was explained in detail.

    • An orientation workshop for Data Managers and Assistant Statistical officers was held in Bhopal from 20-22 May 2008. Total 36 statistic related staff attended from 13 districts of Sagar and Jabalpur divisions. The participants consisted of ASOs, BEE, District Data Assistants and State Data Managers. In the three-day workshop, Dr. K.Suresh explained the procedures of basic health statistics in very simple vocabulary. Most of the participants enjoyed his inspiring lectures on importance of their daily work.

    • The second JCC (Joint Coordination Committee) was held in Bhopal on 30th May 2008 (the first one was in March 2007). The annual performance of the project was reviewed. Ms. Yumiko Asakuma, Deputy Director and Ms. Yu Sasaki, from JICA India office attended.

    • Both of them continued to the field in Damoh and confirmed that the ANMs not only appreciated JICA’s technical support, but expressed their enhanced level of confidence and commitment for work.

  • Renovation of the health facilities has been promoted in Damoh and Tikamgarh, applying methods both of HRM (OJT) and TQM.

  • Arrangement of 4 corners in the labour room has been implemented in the health facilities of the project area.

3.1 HRM
Damoh
CHC Pathariya
Labour Room organization

  • JICA Consultants (Ms. Richa Som & Ms. Rohini Jinsiwale) discussed with the BMO,
    Dr. Minj, about the re-arrangement of the Labour Room with all the corners (Labour corner, New Born corner, Observation area and Service delivery corner); display of duty roaster outside the labour room; and to set up examination area in the space adjoining the labour room.

CHC Hatta
Maternity Wing organization

  • The labour room has been renovated with extension and attached toilet.

  • Electricity connections from generator and invertor have been provided.

  • The ANC room and immunization room have been renovated.

Orientation of Maternal Health (MH) card
Damoh
CHC Patera,

  • JICA Coordinator conducted orientation session on the MH card. Out of the 20 SHCs in Patera block, health workers from 14 SHCs attended the orientation session. The participants were oriented as per the guideline for filling up the card.

  • After the orientation, the participants were shown a film on upgraded/organized SHCs of Tendukheda block so that the health workers of Patera block could also organize their respective SHCs. ANMs of SHCs Lohari and Bhatiya informed that they had already updated their centres after receiving the training on core competencies conducted by JICA.

CHC Tendukheda,

  • In Tendukheda block, 11 participants including ANMs, LHVs and SNs were oriented on MH card.

CHC Hatta

  • JICA Consultant oriented the ANMs of Hatta block on the MH card. Each entry, especially for PNC and pregnancy outcome was explained in detail.

CHC Jabera,

  • JICA consultant gave a brief introduction on the MH card as the ANMs reached very late for the orientation. The orientation will have to be taken up once again.

Tikamgarh

  • JICA Consultants oriented health workers of Baldevgarh, Badagaon, Niwari, Prithvipur and Palera blocks on MH card.

  • About 90% of the health workers attended the orientation in all the 5 blocks.

  • MH card orientation in Jatara block would be held soon.

3.2 TQM
Damoh
CHC Tendukheda

  • The store keeper informed that he was struggling with the arrangement of the materials in the store room, shifted to other room. JICA Coordinator gave suggestions for arranging the store and shared a draft of the layout with him.

CHC Hatta

  • The coloured bins had reached the CHC but were not being used.

  • JICA consultant explained the use of each coloured bin and where they should be placed.

  • Based on the contents of the bins, it was worked out that each CHC needs 3 sets.

  • Making deep burial pits is difficult. Hence an alternative needs to be worked out.

CHC Jabera

  • ANC clinic needs renovation.

  • The Male Supervisors and BEE expressed their interest to know the basics of the trainings given to the ANMs, so that they can supervise.

 

Tikamgarh
CHC Badagaon

  • JICA consultants gave orientation to the staff on labour room and maternity wing layout.

  • They suggested that the labour room, ANC clinic room and the maternity wing could be shifted to the adjacent building occupied by the administration section. The administrative office could be shifted in the current maternity wing.

CHCs Niwari and Prithvipur

  • JICA consultants gave technical inputs for management of Bio Medical Waste and shared the designs of deep burial and sharp pits with the BMOs.

  • The JICA team shared copies of application forms to be submitted to the PCB and explained how to fill them. They also helped the BMOs in calculating the amount of fees they would have to pay to the PCB.

  • After the Quality Circle meeting conducted by JICA consultants at CHC Niwari on 8th April 2008, the BMO, Dr. Vajpayee had issued a letter to the health workers, instructing them organize their SHCs properly as per the guidelines given by JICA team.

  • In response to the letter, the ANM of SHC Orchha, Ms. Uma Khare informed that she had got the SHC painted with plastic paint, using the untied funds.

 

3.3 HMIS
Orientation workshop for Data Managers & Assistant Statistical Officers

  • An orientation workshop on ‘Health Indicators, Validation Rules and Reporting procedures’ was held from 20-22 May 2008 at the IEC Bureau, Bhopal.

  • Thirty-six participants including ASOs, District Data Assistants and BEEs from 13 districts of Sagar and Jabalpur divisions attended the workshop.

  • Dr.K.Suresh, an eminent Public Health Specialist, was invited as a Resource Person to facilitate the workshop.

  • Dr.S.K.Shrivastava, Joint Director, RCH, was present for one and a half days during the workshop. He made a presentation on ‘NRHM and the importance of reporting’ and emphasized on the State’s concern with regards to Reporting and Validation.

CHC Hatta

  • The DEO informed that he is receiving the reports on time.

  • JICA Consultant suggested the DEO to initiate the practice of Supervisors submitting the Form 6 of each SHC and doing spot checks to assess the level of filtration of data from SHC to sector level.

  • JICA consultant asked the DEO to encourage only one type of reporting, as the ANMs were also recording in separate registers.

Consultation on proxy indicators

  • JICA Consultants discussed with I/C BMO Dr. Parandi on Proxy indicators for CHC/PHC.

  • Dr. Parandi suggested 11 very useful indicators, which need to be discussed in a larger group and finalized.

Meeting with CMHO and DPM, Damoh

  • JICA consultants met the CMHO, Dr. Ashok Jain and DPM, Mr. Jaimon Thomas, and discussed the following issues:

  • The need to have clean baby wraps to avoid infections in the neonates. The CMHO agreed and said that the resources could be managed through the DHS or RKS.

  • The team shared all the posters developed by JICA/MP-RH Project, with the CMHO and the DPM. They were very appreciative of the posters.

  • There was a verbal agreement by the CMHO and the DPM on three days of mandatory hospital stay for the JSY beneficiaries.

 

3.4 IEC/BCC
Tikamgarh

  • Follow up of Husbands’ Quiz was done in Sujanpura village.

  • JICA Coordinator gave on the job coaching to the health workers (4 MPWm, 1 ANM) on conducting the follow up.

  • Seven participants, who had attended the quiz program were interviewed.

  • The follow up revealed that there was an increase in the knowledge of the participants from 20% to 60% after the program.

Damoh

  • Follow up of Husbands’ Quiz was done in Taradehi village.

  • The JICA Coordinator followed the same methodology as that used in Tikamgarh.

  • Five health workers (3 MPWm, 1 ANM and 1 Male Supervisor) were present during the follow up.

  • The Male Supervisor, Mr. Pradeep Panika commented, “Sir is tarah ke program ki gaon mein bahut demand hai” [Sir, this kind of program is in great demand in the villages].

 

Meeting with DPM and IEC Consultant, Tikamgarh

  • The IEC Consultant, Mr. Sunil Verma has requested JICA Coordinator to provide support for preparing the activity plan for the district.

  • He also suggested that if IEC Bureau, Bhopal would discuss with the District IEC consultants before developing any IEC material, then local needs could be taken care of.

  • Mr. Sunil also suggested that it would be useful if IEC Bureau could send a copy of the letter to the IEC consultants, along with the CMHO regarding the materials sent.

  • The DPM, Ms. Sangita Verma, informed that they had included 6 Husbands’ Quiz programs for each block in the district PIP.

 

Meeting with Field Publicity Officer, Sagar

 

  • JICA IEC Coordinator met the Field Publicity Officer, Mr.K.N.Pathak, to explore possibilities for collaboration with the health department for IEC activities.

  • Mr. Pathak informed that they had lot of material. He has requested support for the following:

    • Coordination by the districts in the monthly activity plan

    • Logistic arrangement by the districts for their field officers

    • Financial support to convert the audio-video cassettes into CDs

 

4. Meetings
1) The Joint Coordination Committee (JCC), 30th May 2008.

  • The DoHFW officials present for the JCC included Dr. K.K.Shukla, Director, Health & Family Welfare; Dr. S.K.Shrivastava, Joint Director, RCH; Dr. Mittal, Joint Director; Deputy Directors - Dr. L.B.Asthana and Dr. Bhushan Shrivastava; Dr. Archana Mishra, MCH Consultant, NRHM; Mr. Atul Kulshreshtha, State Program Manager, NRHM/RCH; and other State Consultants for NRHM

  • The district team comprised of CMHO, Civil Surgeon, DPM from 5 districts of Sagar division.

  • Ms. Asakuma and Ms. Yu Sasaki from JICA ID office, New Delhi, attended the meeting. The entire JICA/MP-RH Project team was also present.

  • Dr. Yoichi Yamagata, Project Manager, MP-RHP made a presentation for the Annual Review of the project and the progress made during the period January 2007 – March 2008.

  • The Project activities went in concert with the State Policy and the commitment and vision of NRHM. “Quality” was the common concern.

  • Feedback was given by the districts on their observation and experience of the project activities.

 

Points of Advocacy to the State

  • Quality Assurance initiatives like pre-and-post tests etc. should be taken up at the State as well as Regional level.

  • Placing BPHNs (Block Public Health Nurses) in all blocks would be useful in ensuring delivery of quality services and monitoring of the same.

  • The State should develop a mechanism for dissemination of various knowledge products developed by the Project.

 

5. Visitors

  • Ms. Yumilo Asakuma, Deputy Resident Representative and Ms. Yu Sasaki, JICA India office, visited JICA/MP RH Project office, Bhopal from 30-31 May 2008. The purpose of the visit was to attend the JCC on 30th May 2008 and assess the progress of the project activities.


Field visit to Damoh

  • JICA TQM Consultant, Mr. Fareed Uddin accompanied Ms. Asakuma and Ms. Sasaki to Damoh district.

  • The team visited SHC Kishenganj of block Pathariya, where they had good interaction with the Sarpanch, ANM and ASHA. They were quite satisfied with the well organized SHC and the records maintained.

  • The team then had a meeting with the DPM, Mr. Jaimon Thomas in his office. The DPM informed that the inputs provided by JICA team were very important and they are working in close association. Ms. Asakuma suggested him that he should make a proper presentation on the progress of the activities in future meetings.

  • The team finally visited CHC Pathariya. The DPM also accompanied and showed around the entire CHC. Posters developed by JICA/MP-RHP were well displayed; labour room was well organized; store was properly maintained. They were specially impressed by the record keeping.

  • The team had good interaction with the I/C BMO, Dr. Rawat and all the health workers present in the block meeting.

  • Ms. Asakuma asked the ANMs how they felt after working with JICA team and attending the training. The ANMs replied that the training was very practical oriented and they were able to learn many new things which they had not practiced earlier. They had also started conducting ANC clinics regularly.

  • Ms. Asakuma then asked whether the ANMs would continue working in this manner even after JICA project is completed. To this, the ANMs responded, “Earlier we were not in a habit of working. But now, working is a part of our habit and so we will not leave it. JICA project has given us the confidence which we were lacking”.

3.1 HRM
Tikamgarh
Duty Roaster planning (PHC Jeron)

  • JICA team discussed about the advantages of developing the PHC as a delivery care centre with the staff and explained how planning the duty for each staff would enable them (the staff) to provide better quality services.

  • JICA team facilitated the process of mapping out the human resources available at the PHC and preparing the duty roaster for each staff.

Handholding (PHC Teherka and PHC Orchha)

  • Five out of the 9 ANMs at PHC Teherka and 3 our of 20 ANMs at PHC Orchha had not been trained on core competencies.

  • JICA consultants demonstrated the correct method of antenatal check up and procedure for filling up the ANC cards to the ANMs.

  • The ANMs practiced the ANC skills on the 8 ANC cases available at the PHC.

3.2 TQM
Tikamgarh
Microplanning (PHC Jeron, PHC Teherka, PHC Orchha)

  • JICA team facilitated the Microplanning exercise and explained the steps in detail.

  • The participants (health workers) prepared the map as well as the microplan for their respective work areas.

QC meeting at CHC Niwari

  • JICA team conducted the Quality Circle meeting at the CHC for the first time.

  • The participants included Dr. V.K.Vajpai, BMO; Medical Officers; DEO; Lab Technicians; BEE; Health supervisors and health workers of Niwari block.

  • The BMO commented that JICA’s technical inputs through this kind of meeting would be very useful for improving the skills and work performance of his staff as well as himself.

  • The next QC meeting has been scheduled for 10th May 2008.

Meeting by SPM (State Program Manager) at PHC Orchha

  • Mr. Atul Kulshreshtha, State Program Manager, NRHM, had called a meeting of the health officials of Tikamgarh district at the PHC on 11th April 2008.

  • The participants included the DPM, District DEO, BMOs of blocks Prithvipur, Baldevgarh and NIwari, and SMO of Orchha.

  • The SPM shared the directives received from the Government of India with regards to fund utilization.

  • JICA team shared the guidelines for model SHCs with the SPM and invited him to visit Damoh district to observe the SHC models developed

  • The District DEO commented that JICA intervention in HMIS has made 3 major differences in their reporting. They are:

    • The reports are being submitted on time

    • Since the source of information is being provided, most of the cells are being filled

    • The bilingual form is helping the reports being filled correctly

Meeting with the District Collector, Tikamgarh

JICA team (Dr. Yoichi Yamagata and Mr.S.Fareed Uddin) met the District Collector,
Mr. Vivek Porwal, I.A.S., on 24th April 2008. The purpose of the meeting was to explain the position of the project and that ASHA training was not within the scope of the project. Though, the project would be willing to provide the training (TOT) to the resource persons identified by the district. Dr.Yamagata also suggested that retired personnel from Health department can be head hunted for this purpose, to which the Collector requested JICA team to facilitate the selection process.

 

District Meeting – ICDS - 4 

  • The meeting was held on 24th April 2008 to finalise the plan for Tikamgarh district for the year 2008-09 for the World Bank assisted ICDS – 4 project.

  • The participants included CDPOs and ICDS Supervisors of all the blocks.

  • JICA Coordinator attended the meeting and gave suggestions during the preparation of the plan.

  • The DPO, Mr. Brajesh Tripathi, informed that the 12th Bal Sanjivani drive would begin from 15th May – 15th June 2008. He requested if JICA could provide relevant IEC materials like posters, booklets etc. so that these materials could be used as counseling tools.

  • The DPO also requested if JICA could give support for providing awards to Best Mothers during Gaud Bharai sessions.

  • JICA/MP-RH Project has agreed to do so in the intervention areas.

4. Meetings

  • As a follow up of the meeting with the Health Commissioner on 30th April 2008, a meeting was held with Dr. K.K.Shukla, Director, PH&FW at JICA office on 1st May 2008. About seven action points were listed down where JICA was requested to provide technical support.

  • Meeting of JICA team (Dr. Yoichi Yamagata, Mr.S.Fareed Uddin, Dr. Aboli Gore and
    Ms. Richa Som) with Ms. Alka Upadhyay, Health Commissioner, DoHFW, at EPCO office on
    30th April 2008. The purpose of the meeting was to apprise the Health Commissioner about the JICA/MP-RH Project interventions and the progress made till date. The meeting was also attended by Dr.K.K.Shukla, Director, PH&FW and Dr.S.K.Shrivastava, Joint Director, RCH.

  • JICA team (Mr. S.Fareed Uddin and Dr. Aboli Gore) attended the meeting chaired by
    Ms. Alka Upadhyay, Health Commissioner, DoHFW, at EPCO office on 19th April 2008.
    The purpose of the meeting was to share the experiences of the NGO Nivedita in operationalising the Public-Private-Partnership for Janani Express call centre.

  • Meeting of JICA team (Dr. Yoichi Yamagata, Mr. S. Fareed Uddin and Ms. Richa Som) with
    Dr. Savita Jain, State Facilitator IEC of NHSRC on 17th April 2008 to explore the possibilities of partnership with NHSRC on IEC issues.

  • Meeting of Dr. Aboli Gore, HRM Consultant, JICA with Dr. Archana Mishra, MH Consultant, DoHFW on 17th April 2008 to discuss about organization of ToT for retired nursing staff.

5. Visitors

  • Ms. Yu Sasaki, JICA India office, visited JICA/MP RH Project office, Bhopal from
    21st -23rd April 2008 to understand the project activities.

  • Three members of the HMIS team of NHSRC, New Delhi, namely, Dr. Arthur Heywood, International HMIS Expert; Dr. Sandeep Sahay, Advisor, HMIS and Mr. John, Research Scholar Health Informatics, University of Oslo, visited JICA/MP-RH Project office, Bhopal on

1. Summary

  • JICA/MP-RH Project sponsored and facilitated participation of State officials (Two doctors) as well as district staff (1 Staff Nurse and 1 ANM) in training courses organized in JICA in Japan on Safe Motherhood.

  • New initiatives started by introducing orientation of the CHC and PHC staff on Labour Room arrangement, use of New Born Care equipments and Early initiation of Breastfeeding for deliveries conducted in the institution.

  • A structured Gaud Bharai activity was demonstrated to CDPO, ICDS supervisor, 9 Anganwadi workers and 9 Anganwadi helpers in Sarra sector of Damoh district to build their capacity to conduct better Gaud Bharai sessions at their respective centres.

  • NRHM review meeting for Sagar division was held to assess the progress of the activities and status of fund utilization by the districts.

  • Ms. Umi KOJIMA, Country Officer, JICA India visited the JICA/MP-RH Project site. During her stay, the Project Plan of Operation was revised covering all the 4 components.

3.1 HRM
Training in Japan

  1. Workshop on Safe Motherhood Strategy for Asia – Improvement of Conducive Environment for Mothers at the Community Level, 4th – 23rd February 2008, Tokyo, Japan organized by JICA and JOICFP (Japanese Organization for International Cooperation in Family Planning).

  • JICA/MP-RH Project facilitated participation of Dr. L.B.Asthana, Deputy Director, Training and Dr. J.P.Khare, Deputy Director, RCH/JICA of DoHFW, Madhya Pradesh in the training.

  • In all 14 participants (Maternal and Child Health Program Managers) from 7 countries including India participated in the workshop.

  • The objective of the workshop was to develop good practices of community-operated Maternal and Child Health (MCH) Strategies for the improvement of Safer Motherhood status in the South and South East Asian countries.

  • The Indian participants prepared an Action Plan at the workshop and shared with the Project office and the State when they came back.

  1. Midwifery Course for Safe Motherhood, 28th January – 1st March 2008, Osaka, Japan, organized by JICA and INFJ (International Nursing Foundation of Japan).

  • JICA/MP-RH Project facilitated participation of Ms. Jasmin Ram, Staff Nurse, Hatta, Damoh and Ms. Rashmi Sen, ANM, Panna in the training. Both the participants belong to the Project Area (Sagar division).

  • In all 10 participants (Midwives working at clinical level) from 3 countries including India participated in the training.

  • The objective of the course/training was to develop an understanding among the participants on.

    • Prenatal care

    • Maternal and Child Health (MCH) and Role of Midwives

    • In-service training for Midwives

    • Health Education for pregnant women

    • Roles of Maternity Clinics, Public Health Centres and Midwives and their partnership structure

    • Ability to analyze issues of MCH and to provide solutions

  • Both the participants from India commented after coming back that although most of the above topics had been taught to them in India, it was useful to observe how to put them into practice in Japanese ways.

Damoh
Orientation on New Born Care

  • JICA Training Coordinator oriented the District Hospital staff (7); CHC staff of Pathariya (11) and Jabera (9); mini PHC Abhana (6) and PHC Sarra (5) on use of New Born Care equipments (Radiant Warmer, Suction pump, Ambu bag) and New Born Care (Baby wrapping).

Orientation on Breastfeeding

  • JICA coordinator oriented the District Hospital staff (7); CHC staff of Pathariya (11); mini PHC Abhana (6) and PHC Sarra (35 including health and ICDS staff, pregnant women from the village) on early initiation of breastfeeding.

  • Demonstration using a flip book, doll and breast model was done to explain the correct position of mother and baby; and attachment of the baby to the mother’s breast. A video film on “Breast Crawl” technique was shown to all the participants.

  • At mini PHC Abhana, breastfeeding within half an hour after delivery was successfully initiated on a newly delivered case using the Breast Crawl technique.

Exposure visit of JICA field staff

  • JICA field coordinators based in Tikamgarh district visited Damoh from
    12th -13th March 2008 to observe the good practices in JICA intervention blocks and replicate them in their respective work areas. They visited CHC Tendukheda, and SHCs Tejgarh, Taradehi, Harrai in Tendukheda block. In Pathariya block, they visited CHC Pathariya and SHCs Kishenganj and Narsingarh.

3.2 IEC/BCC
Panchayat Meeting, Damoh

  • CEO, Mr. N.S.Bhuaraya and BMO, Dr. Tulsa Thakur had called a meeting of all the Surpanch and Panchayat Secretary of Sarra sector at Tendukheda on 29 March 2008.

  • The main objective of the meeting was to discuss on the financial guidelines for the next fiscal year 2008-09 and to inform them about the various government schemes.

  • JICA IEC Coordinator attended the meeting and had a brief discussion with the participants about the project activities.

  • A tentative plan was made to conduct a meeting of the Panchayat members of Sarra sector on importance of ante natal care and various government schemes, specifically on health.

Gaud Bharai
    Damoh

  • A structured Gaud Bharai activity was conducted at PHC Sarra of block Tendukheda and demonstrated to the health and ICDS field staff.

  • AWWs and AWHs of 9 AWCs out of 10 were present during the session. The ICDS Supervisor of Sarra sector and CDPO of Tendukheda block were also present throughout the session. Eight pregnant women of Sarra village were called for demonstration.

  • The structured method of carrying out the session and all the steps were discussed in detail with the AWWs and helpers. AWW of Sarra AWC was asked to conduct the mock Gaud Bharai session as per the discussion.

  • JICA Documentation Coordinator then conducted the entire session and demonstrated the method.

4. Meetings

  • Review meeting of Sagar division – 19 March 2008

    1. The State DoHFW team comprising of Ms. Alka Upadhyaya, Health Commissioner ; Dr. KK Shukla, DPH; Dr. SK Shrivastava, JD (NRHM/RCH); Dr. AN Mittal, JD; Dr. Archana Mishra, MH Consultant; Dr. Jayshree Chandra, Dy. Director – Child Health and Mr. Shivhare, Exe. Engineer conducted the review meeting for Sagar division on 19 March 2008 at the Sagar RJD office.

    2. The Sagar division participants consisted of Dr. K.K.Tamrakar, RJD; Chief Medical and Health Officers, as well as, District Program Managers of all the 5 districts.

    3. The major focus of the review meeting was on the expenditure of funds allotted under NRHM and the activities carried out in the districts.

    4. Dr. Yoichi Yamagata, Project Manager, JICA/MP-RH Project gave a brief account of the project activities in Tikamgarh and Damoh and shared about the initiations in Sagar, Panna and Chhatarpur districts from April 2008.

    5. For the conduction of SBA trainings, JICA consultants suggested to hire services of the retired health staff as trainers and shared the list of identified persons.

    6. Dr. S.K.Shrivastava and Dr. Archana Mishra agreed to the point of engaging the retired health staff for SBA training and requested JICA Project to conduct the TOT (Training of Trainers).

  • Meeting with Dr. Archana Mishra at JICA office – to share the package developed for SBA training, 19 March 2008.

  • Feedback by Ms Rashmi Sen, ANM, Panna on training attended in Japan – 5 March 2008.

  • Presentation by Dr. L.B.Asthana, Deputy Director, Training and Dr. J.P.Khare, Deputy Director, RCH/JICA of DoHFW, Madhya Pradesh, on training attended in Japan – 12 March 2008.

  • Meeting of Dr. Yoichi Yamagata Yoichi with Dr. Sundararaman, NHSRC, New Delhi on 18 March 2008 to share the progress of the project activities.

5. Visitors

  • Ms. Umi KOJIMA, Country Officer, JICA India visited the JICA/MP-RH Project site from
    12th to 14th March 2008 and again from 24th to 26th March 2008. The purpose of her visit was to collect information about the Project activities in the fiscal year 2007-08, review the PDM (Project Design Matrix) and PO (Plan of Operations) and to discuss the PO for the fiscal year 2008-09.
    Ms. Yumiko TANAKA, Regional Advisor on Gender and Monitoring/Evaluation, JICA, Bangkok, visited Madhya Pradesh to observe addressed through the JICA/MP-Reproductive Health Project and also the initiatives taken by the State through various departments.
    Visit to Sehore – 18 March 2008
    Ms. TANAKA visited Sehore district and had a meeting with CMHO and DPM who shared information about health system in MP, various schemes provided by the State Government and service delivery mechanism.
    CHC Ichhawar – Ms. TANAKA had interaction with BMO and other staff, to understand the working environment and to get an idea of the health system in MP and had opportunity to interact with the female patients admitted in the CHC.
    Visit to Berasia, rural Bhopal – 19 March 2008
    Ms. TANAKA also visited the Anganwadi Centre (outlet of Department of Women & Child Development) of Berasia block (rural) of Bhopal district and SHC to assess the level of empowerment of field staff.
